Resumo: The gut microbiota plays an important role in providing human health barrier to colonization of pathogens (competitive exclusion), to exert important metabolic functions and stimulate the immune system. Lactobacillus and Bifidobacterium are the two most important genera for the maintenance of health. The intestinal microbiota may be qualitatively and quantitatively altered by the type of food eaten, called nutritional modulation, and the use of antiinflammatory drugs, laxatives and antibiotics. The main objective of this study was to determine the influence of either type of feeding and delivery on both composition and evolution of the gut microbiota of Bifidobacterium spp., Lactobacillus spp. Candida spp. Clostidium spp., Escherichia coli, fungi and total aerobic and anaerobic, in infants. The influence of diet and type of delivery in 68 neonates aged between seven and 21 days was evaluated. It was found that the type of delivery influence counts of lactobacilli, which were higher in those born vaginally. Counts of lactobacilli in infants fed LPM and LM and born vaginally were significantly higher (p <0.05) than those born by caesarean, suggesting a possibility of transference of the micro biota from mother to child. Lactobacilli counts were below the detection level of the method (1x102 CFU/g) in 44% of individuals fed with BLH?s milk, born vaginally and in 81% of those born caesarean section. Yeasts were found in all samples, regardless of type of delivery or feeding in numbers ranging from 1x105 to 1x1011. Candida albicans (30%) and C. parapsilosis (28%) were predominant in both types of delivery, independently of type of feeding with the exception of the micro biota of infants who were born vaginally and were fed with LPM, when prevailed the yeast of the genus Trichosporon sp. There was no significant influence of type of delivery, in counts of anaerobic microbiota. Another objective was to detect the presence and identify the fungal microbiota of human milk and anatomical sites in women and infants. Eighty-one per cent of 64 samples were positive for the presence of fungi, with a prevalence of Candida albicans (73%) followed by Candida parapsilosis (15.4%). Similar profiles to those observed in the total samples were found in milk, breast and oral cavity, suggesting a relationship between the skin infection of the mother and the infant with the milk ingested. The virulence of Candida species isolated was determined by testing the production of proteases and 100% were strongly positive, indicating a high degree of infectivity. Afterwards, it was investigated by means of PCR and PFGE, the occurrence of E.coli enterophatogenic (EPEC) e enteraggregative (EAEC) virulence genes in 39 E. coli isolates from new-born children faecal samples and rectal swabs. The targets selected for EPEC were the genes encoding for intimin adherence protein (eae), adherence factor (EAF) plasmid, fimbriae bundle forming pili (bpf). The genes target for EAEC were the plasmid for aggregative adherence (pAA) and its regulon (AggR). Both EAEC virulence targets were identified in one of the samples (2,6%) in neonates fed with milk formula, prepared by the maternity personnel. It is concluded that besides the type of delivery and breast-feeding, another important determinant of colonization of the intestine of infants are the hygienic conditions in food preparation and in new-born cares. It should be taken corrective actions related to the conditions of manipulation of milk for children, and prioritize the supply of breast milk especially in the first days of life when the secretion of colostrum is rich in antibodies and phagocytes.

Bifidobacterium e Lactobacillus s?o os dois g?neros mais importantes para a manuten??o da sa?de. A microbiota intestinal poder? ser qualitativa e quantitativamente alterada pelo tipo de alimento ingerido, chamada modula??o nutricional, e pelo uso de anti-inflamat?rios, laxantes e antimicrobianos. O objetivo do trabalho foi avaliar a composi??o e evolu??o da microbiota intestinal de crian?as rec?m nascidas e relacionar ao tipo de amamenta??o, de parto, com a presen?a de Bifidobacterium spp., Lactobacillus spp., Clostridium spp., Escherichia coli e fungos, al?m da determina??o de aer?bios e anaer?bios totais. Foi avaliada a influencia da alimenta??o e do tipo de parto em 68 neonatos com idade entre sete e 21 dias. Verificou-se influ?ncia do tipo de parto nas contagens de lactobacilos, que foram maiores naqueles nascidos de parto normal. Contagens de lactobacilos em lactentes alimentados com leite em p? modificado (LPM) e leite do peito (LM) e nascidos de parto normal foram significativamente superiores (p<0,05) aquelas dos nascidos de parto cesariano, sugerindo uma possibilidade de transfer?ncia da microbiota da m?e para a crian?a. As contagens de lactobacilos estiveram abaixo do n?vel de detec??o do m?todo (1x102 UFC/g) em 44% dos indiv?duos alimentados com leite do banco de leite humano (BLH) e nascidas de parto normal e em 81% daquelas nascidas de parto ces?reo. Leveduras foram encontradas em todas as amostras, independentemente do tipo de parto ou aleitamento em n?meros que variaram de 1x105 a 1x1011. Candida albicans (30%) e C. parapsilosis (28%) predominaram em ambos os tipos de parto independemente do tipo de aleitamento com excess?o da microbiota de lactentes que receberam LPM nascidos de parto normal, quando predominou a levedura do g?nero Trichosporon sp. N?o foi observada influencia significativa do tipo de parto, nas contagens da microbiota anaer?bica. Outro objetivo do trabalho foi detectar a presen?a e identificar a microbiota f?ngica de leite humano e de s?tios anat?micos em mulheres e lactentes. A virul?ncia das linhagens de levedura encontradas foi avaliada atrav?s de testes de determina??o da atividade proteol?tica. De 64 amostras avaliadas, 81% foram positivas para a presen?a de fungos, com preval?ncia de Candida albicans (73%) seguida de Candida parapsilosis (15,4%). Perfis semelhantes aos verificados no total de amostras foram encontrados no leite, nas mamas e na cavidade oral, concluindo-se que existe a possibilidade da transfer?ncia da infec??o cut?nea da m?e para o lactante com o leite ingerido. A virul?ncia das esp?cies de Candida isoladas, foi determinada pelo teste de produ??o de proteases sendo que 100% foram fortemente positivas, indicando alto grau de infectividade. Posteriormente, investigou-se a presen?a de genes de virul?ncia de E. coli enteropatogenica (EPEC) e enteroagregativa (EAEC) em 39 isolados de amostras fecais e de swabs retais dos rec?m nascidos atrav?s das t?cnicas de polimerase chain reaction (PCR) multiplex e pulsed field gel eletrophoresis (PFGE). Os alvos selecionados para EPEC foram os genes que codificam a adesina intimina (eae), fator de ader?ncia (plasm?deo EAF) e f?mbria ?bundle forming pillus? (bpf). Os genes alvo para EAEC foram o plasm?deo de ader?ncia agregativa (pAA) e regulon (AggR). Foram identificados dois alvos de virul?ncia para EAEC em uma das amostras (2,6%) de neonatos alimentados com leite em p? modificado preparado em lact?rio. description The gut microbiota plays an important role in providing human health barrier to colonization of pathogens (competitive exclusion), to exert important metabolic functions and stimulate the immune system. Lactobacillus and Bifidobacterium are the two most important genera for the maintenance of health. The intestinal microbiota may be qualitatively and quantitatively altered by the type of food eaten, called nutritional modulation, and the use of antiinflammatory drugs, laxatives and antibiotics. The main objective of this study was to determine the influence of either type of feeding and delivery on both composition and evolution of the gut microbiota of Bifidobacterium spp., Lactobacillus spp. Candida spp. Clostidium spp., Escherichia coli, fungi and total aerobic and anaerobic, in infants. The influence of diet and type of delivery in 68 neonates aged between seven and 21 days was evaluated. It was found that the type of delivery influence counts of lactobacilli, which were higher in those born vaginally. Counts of lactobacilli in infants fed LPM and LM and born vaginally were significantly higher (p <0.05) than those born by caesarean, suggesting a possibility of transference of the micro biota from mother to child. Lactobacilli counts were below the detection level of the method (1x102 CFU/g) in 44% of individuals fed with BLH?s milk, born vaginally and in 81% of those born caesarean section. Yeasts were found in all samples, regardless of type of delivery or feeding in numbers ranging from 1x105 to 1x1011. Candida albicans (30%) and C. parapsilosis (28%) were predominant in both types of delivery, independently of type of feeding with the exception of the micro biota of infants who were born vaginally and were fed with LPM, when prevailed the yeast of the genus Trichosporon sp. There was no significant influence of type of delivery, in counts of anaerobic microbiota. Another objective was to detect the presence and identify the fungal microbiota of human milk and anatomical sites in women and infants. Eighty-one per cent of 64 samples were positive for the presence of fungi, with a prevalence of Candida albicans (73%) followed by Candida parapsilosis (15.4%). Similar profiles to those observed in the total samples were found in milk, breast and oral cavity, suggesting a relationship between the skin infection of the mother and the infant with the milk ingested. The virulence of Candida species isolated was determined by testing the production of proteases and 100% were strongly positive, indicating a high degree of infectivity. Afterwards, it was investigated by means of PCR and PFGE, the occurrence of E.coli enterophatogenic (EPEC) e enteraggregative (EAEC) virulence genes in 39 E. coli isolates from new-born children faecal samples and rectal swabs. The targets selected for EPEC were the genes encoding for intimin adherence protein (eae), adherence factor (EAF) plasmid, fimbriae bundle forming pili (bpf). The genes target for EAEC were the plasmid for aggregative adherence (pAA) and its regulon (AggR). Both EAEC virulence targets were identified in one of the samples (2,6%) in neonates fed with milk formula, prepared by the maternity personnel. It is concluded that besides the type of delivery and breast-feeding, another important determinant of colonization of the intestine of infants are the hygienic conditions in food preparation and in new-born cares. It should be taken corrective actions related to the conditions of manipulation of milk for children, and prioritize the supply of breast milk especially in the first days of life when the secretion of colostrum is rich in antibodies and phagocytes.
