Implantação do serviço de PrEP no hospital universitário de Sergipe: análise das dificuldades e perspectivas

Resumo: The human immunodeficiency virus (HIV) is the cause of one of the world's major epidemics. epidemics. According to a report by UNAIDS, it is estimated that more than 35 million people died since the beginning of the HIV-AIDS epidemic. In Brazil, HIV prevalence is approximately 0.4% in the entire population, and in 2017, 15,653 new cases were registered in 2017. This prevalence increases in certain population subgroups such as transgender people, drug users, men who have sex with men (MSM) sex workers, and serodiscordant couples, respectively. PrEP is the combination of the antiretroviral drugs tenofovir and emtricitabine (TDF/FTC), indicated for HIV-negative HIV-negative people who are at high risk of contracting HIV. Several studies have proven the effectiveness of PrEP. Objective: To verify the difficulties and perspectives of the implementation of the reference service of Prophylaxis Pre-Exposure Prophylaxis reference service at the Sergipe University Hospital. Justification: it is important to consider PrEP as one more tool in combating and controlling HIV, being a strategy that needs to have its implementation studied, especially in pioneer sites. The Hospital University Hospital of the Federal University of Sergipe is a state reference for people who prevention of HIV/AIDS through Pre-Exposure Prophylaxis. In this way, the study of the difficulties of implementation is essential for the improvement and expansion of PrEP programs. Methods: Cross-sectional, descriptive, and qualitative study. Conducted with 66 participants, including health professionals, medical students and users of PrEP in the HU-UFS HU-UFS in the period between April and September 2019. The data analysis was performed according according to the sample size, using measures of central tendency and Fisher's exact test for the quantitative data and discourse analysis for the qualitative questions. Results: A mean age of the participants was 33.5 years, the mean age among the male was 30.2 years and among females 41 years. The majority (69.2%) of the participants are male, homosexual, single, brown, and have studied for 12 years or more. Among the females, all identified themselves as female, heterosexual, had only one fixed sexual partner, and studied for up to 11 years. The main concern was to develop any side effects. The majority of the participants did not encounter any difficulty in accessing the service. The biggest expectation of the users was the increase in disclosure (30.7%). Among the multiprofessional team the main points approached during health health education were condom use and adherence to treatment. Conclusions: Health education education and continuing education of the professionals that make up the service is a key component in the component in the implementation of PrEP. It is suggested, with this work, to expand the training to other professional professional categories besides the core of PrEP, as well as expand the dissemination of the service

Essa prevalência aumenta em determinados subgrupos populacionais como pessoas transexuais, usuários de drogas, homens que fazem sexo com outros homens (HSH), profissionais do sexo, e casais sorodiscordantes, respectivamente. A PrEP é a combinação dos antirretrovirais tenofovir e emtricitabina (TDF/FTC), indicada para pessoas soronegativas que possuem alto risco de contrair HIV. Diversos estudos comprovam a eficácia da PrEP. Objetivo: Verificar as dificuldades e perspectivas da implantação do serviço de referência de Profilaxia Pré-Exposição no Hospital Universitário de Sergipe. Justificativa: é relevante considerar a PrEP como mais uma ferramenta no combate e controle ao HIV, sendo uma estratégia que necessita ter sua implantação estudada, principalmente em locais pioneiros. O Hospital Universitário da Universidade Federal de Sergipe é referência estadual para as pessoas que fazem prevenção combinada ao HIV/AIDS por meio da Profilaxia Pré-Exposição. Dessa forma, o estudo das dificuldades de implantação é fundamental para a melhoria e ampliação dos programas de PrEP. Métodos: Estudo transversal, descritivo e qualitativo. Realizado com 66 participantes, entre eles profissionais de saúde, estudantes de medicina e usuários da PrEP no HU-UFS no período entre abril e setembro de 2019. A análise dos dados foi feita de acordo com o tamanho da amostra, sendo utilizadas medidas de tendência central e teste exato de Fisher para os dados quantitativos e análise do discurso para as perguntas qualitativas. Resultados: A média de idade dos participantes foi de 33,5 anos, a idade média entre os indivíduos do sexo masculino foi 30,2 anos e, entre os do sexo feminino de 41 anos. A maioria (69,2%) dos participantes são do sexo masculino, homossexuais, solteiros, pardos e estudaram por 12 anos ou mais. Entre os do sexo feminino, todas se identificaram como mulheres, heterossexuais, possuíam apenas um parceiro sexual fixo, e estudaram por até 11 anos. O principal anseio foi desenvolver algum efeito colateral. A maioria dos participantes não encontraram nenhuma dificuldade para acessar o serviço. A maior expectativa dos usuários era o aumento da divulgação (30,7%). Entre a equipe multiprofissional os principais pontos abordados durante educação em saúde eram uso de preservativos e adesão ao tratamento. Conclusões: A educação em saúde e a educação continuada dos profissionais que compõem o serviço é um componente- chave na implantação da PrEP.
