Tratamento cirúrgico para pacientes diabéticos tipo 2 não obesos. Resultados preliminares.

Resumo: Diabetes mellitus is a disease considered an epidemic by the World Health Organization. Type 2 diabetes was controlled successfully with bariatric surgery in morbidly obese patients, becoming regarded as surgical treatment for patients with type 2 diabetes and obesity. In order 36 to get the same metabolic benefits of bariatric surgery, but no significant weight loss, began this research and the preliminary results are presented. The patient agreed with the inclusion criteria underwent bypass surgery duodenal-ileal 250 cm from the ileocecal valve (Marchesini modified technique), patient 1 had diabetes for 13 years and used 28 units of insulin and presented the preoperative glycemic control: fasting blood glucose of 218mg/dl, 10.2% glycated hemoglobin, basal C-peptide 1.78 ng/dl and in the twelfth month postoperatively: fasting glucose of 98mg/dl a glycated hemoglobin 6.8%. And the second patient was diabetic for 6 years and used 50 units of insulin and glucose profile presented the preoperative: fasting blood glucose of 152mg/dl, 10.5% glycated hemoglobin, basal C-peptide 1.08 ng / dl and the sixth month postoperatively: fasting blood glucose of 103mg/dl a glycated hemoglobin 6.5%, basal C-peptide 1.97 ng/dl. The patients preoperatively, even with the use of insulin and hypoglycemic agents, blood glucose remained high and glycated hemoglobin values above 7% after surgery controlled the blood glucose levels and reduced glycated hemoglobin only oral hypoglycemic agents. Beyond glycemic control, showed an improvement in the lipid profile. In conclusion the two patients showed improvement in glycemic control with control of diabetes and dyslipidemia after correction of metabolic surgery with no complications and postoperative follow-up

O diabetes tipo 2 foi controlado com sucesso com a cirurgia bariátrica em pacientes com obesidade mórbida, passando a ser considerada como tratamento cirúrgico para paciente com diabetes tipo 2 e obesidade. Com o objetivo de obter os mesmos benefícios metabólicos da cirurgia bariátrica, mas sem significativa perda de peso, iniciou-se a presente pesquisa e são apresentados os resultados preliminares. Os paciente que estavam de acordo com os critérios de inclusão foram submetidos à cirurgia de derivação duodeno-ileal a 250 cm da válvula ileocecal (técnica de Marchesini modificada), a paciente 1 tinha diabetes há 13 anos e usava 28 unidades de insulina e apresentava o perfil glicêmico pré-operatório: glicemia de jejum de 218mg/dl, hemoglobina glicada 10,2%, peptídeo C basal 1,78 ng/dl; e no décimo segundo mês de pós-operatório: glicemia de jejum de 98mg/dl, uma hemoglobina glicada 6,8%. E a paciente 2 era diabética há 6 anos e usava 50 unidades de insulina e apresentava o perfil glicêmico pré-operatório: glicemia de jejum de 152mg/dl, hemoglobina glicada 10,5%, peptídeo C basal 1,08 ng/dl; e no sexto mês de pós-operatório: glicemia de jejum de 103mg/dl, uma hemoglobina glicada 6,5%, peptídeo C basal 1,97 ng/dl. As pacientes que no pré-operatório, mesmo em uso de insulina e hipoglicemiantes, mantinham glicemias elevadas e valores de hemoglobinas glicadas acima de 7%, após a cirurgia controlaram os valores glicêmicos e reduziram a hemoglobina glicada apenas com hipoglicemiantes orais. Além do controle glicêmico, apresentaram melhora no perfil lipídico.
