Resumo: | Introduction: Infections caused by the new coronavirus varies in severity, and may evolve from asymptomatic forms to manifestations that lead to death of patients. In this sense, in its most serious forms, the Covid-19 pandemic has already caused millions of deaths worldwide. As a result, researchers are trying to understand which conditions contribute to the severity of the disease. One of the main common risk factors is obesity, a chronic disease that has shown to contribute to the appearance and worsening of a range of conditions, including cardiovascular disease and diabetes. Objective: Thus, the aim of this study was to carry out a systematic review with meta-analysis on SARS-CoV-2 infections in obese individuals. Methodology: Some pair search was carried out in the PubMed, Web of Science, Scopus and Embase databases, as well as in the gray literature (Google Academic, bioRxiv and medRxiv) between February and April 2021, in order to find publications between the years of 2019 and 2021, but without restriction as to language or place of publication. Results: Following the PRISMA model, 13 articles were included, totaling 9189 individuals. It was found that obese patients are more likely to have hypertension (OR = 1.41 [1.17-1.70]), diabetes mellitus (OR = 1.47 [1.32-1.63]) and lung diseases (OR = 1.57 [1.32-1.87]), as well as progressing to dyspnea (OR = 1.86 [1.27-2.72]) and diarrhea (OR = 1.96 [1.03-3.73]) compared to those without obesity. In addition, higher levels of C-reactive protein (CRP), ferritin and D-dimer were observed among obese individuals. It was also observed that obese patients infected with the new coronavirus were more likely to be admitted in intensive care unit (OR = 1.52 [1.34- 1.73] and intubation (OR = 1.60 [1.37-1.86]) than the non-obese, but with no impact on mortality (OR = 0.98 [0.75-1.28]). Conclusion: Thus, this review reinforces the potential for the infection severity by the new coronavirus among obese patients, highlights that excess body fat is not a definitive predictor for mortality, alerts to the influence of comorbidities on the evolution of Covid-19 and points out that dyspnea and diarrhea, as well as increased levels of CRP, ferritin and D-dimer should be valued in the Emergency Department, especially in people with obesity. |

Nesse sentido, em suas apresentações mais graves, a pandemia da Covid-19 já resultou em milhões de mortes em todo o mundo. Em virtude disso, os pesquisadores tentam entender quais condições contribuem para a gravidade da doença. Um dos principais fatores de risco é a obesidade, uma doença crônica que já demonstrou contribuir para o surgimento e o agravamento de uma série de condições, incluindo doenças cardiovasculares e diabetes. Objetivo: Dessa forma, o objetivo desse estudo foi realizar uma revisão sistemática com metanálise sobre a infecção pelo SARS-CoV-2 em indivíduos com obesidade. Metodologia: Realizou-se uma busca por pares nas bases de dados PubMed, Web of Science, Scopus e Embase, e na literatura cinza (Google Acadêmico, bioRxiv e medRxiv) entre fevereiro e abril de 2021, com filtro para ano de publicação entre 2019 e 2021, mas sem restrição quanto ao idioma ou local de publicação. Resultados: Seguindo o modelo PRISMA, foram incluídos 13 artigos, totalizando 9189 indivíduos. Verificou-se que os pacientes obesos têm maior chance de apresentar hipertensão arterial (OR = 1,41 [1,17-1,70]), diabetes mellitus (OR = 1,47 [1,32-1,63]) e doença pulmonar (OR = 1,57 [1,32-1,87]), bem como de evoluir com dispneia (OR = 1,86 [1,27-2,72]) e diarreia (OR = 1,96 [1,03-3,73]) em comparação àqueles sem obesidade. Além disso, níveis mais elevados de proteína C reativa (PCR), ferritina e D-dímero foram constatados entre os obesos. Observou-se também que pacientes com obesidade infectados pelo novo coronavírus tiveram maior chance de admissão em unidade de terapia intensiva (OR = 1,52 [1,34-1,73] e de intubação (OR = 1,60 [1,37-1,86]) do que os não obesos, porém sem impacto na mortalidade (OR = 0,98 [0,75-1,28]).
