Prevalência de histerectomia pós-parto em maternidade pública do Estado de Sergipe

Resumo: Objective: To collect data on the prevalence, risk factors and main indications of postpartum hysterectomies at a public maternity hospital in the state of Sergipe. Methods: This is a descriptive retrospective cross-sectional study, through the descriptive analysis of records of cases of women submitted to postpartum hysterectomy between January 2012 and December 2016, at Hospital and Maternidade São José, Itabaiana / SE. Including maternal age, parity, gestational age, type of delivery, indications for hysterectomy and outcome of hysterectomy. Results: 23 cases of postpartum hysterectomy (1.70 per 1000 births), 15 (65.0%) after cesarean section (4.26 per 1,000 cesareans) and eight (35%) were found after vaginal delivery (0.80 per 1000 vaginal deliveries). Only one patient was nulliparous. The most common indication was anomalous placentation with eight (34.8%), followed by uterine atony with seven (30.4%) indications. Uterine rupture was found in four patients (17.4%), infection and endometriosis with one (4.3%), hysterectomy indication was not specified in one case. The main complications were the need for hemotransfusion in all women (100%), hypovolemic shock in four (17.39%), disseminated intravascular coagulation and febrile syndrome with two women (8.70) each, two (8.70% ) women were transferred to the Intensive Care Unit. There were no maternal deaths. Conclusions: Postpartum hysterectomy was more prevalent post-cesarean than vaginal delivery. The main cause was anomalous placentation. The main complication was blood transfusion.

Métodos: Trata-se de um estudo descritivo do tipo corte transversal retrospectivo, através da análise descritiva dos registros de prontuários de casos de mulheres submetidas à histerectomia pós-parto entre janeiro de 2012 a dezembro de 2016, no Hospital e Maternidade São José, Itabaiana/SE. Incluindo idade materna, paridade, idade gestacional, tipo de parto, indicações para a histerectomia e desfecho da histerectomia. Resultados: Foram encontrado 23 casos de histerectomia pós-parto (1,70 por 1000 nascimentos), 15 (65,0%) após cesariana (4,26 por 1000 cesarianas) e oito (35%) após parto vaginal (0,80 por 1000 partos vaginais). Apenas uma paciente era nulípara A indicação mais comum foi a placentação anômala com oito (34,8%), seguida de atonia uterina com sete (30,4%) indicações. A ruptura uterina foi encontrada em quatro pacientes (17,4%), infecção e endometriose com uma (4,3%), indicação de histerectomia não foi especificada em um caso. As principais complicações foram a necessidade de hemotransfusão em todas as mulheres (100%), choque hipovolêmico em quatro (17,39%), coagulação intravascular disseminada e síndrome febril com duas mulheres(8,70) cada, duas(8,70%) mulheres foram transferidas para Unidade de Terapia Intensiva. Não houve óbitos maternos. Conclusões: A histerectomia pós- parto foi mais prevalente pós- cesárea do que pós parto vaginal. A causa principal foi placentação anômala.
