A política de descentralização da saúde e suas interfaces escalares

A preocupação central deste artigo é analisar a política de descentralização da saúde que criou o SUS e com ele, uma redistribuição de poder político, fortalecendo o município para operacionalização da política de saúde. Parte-se de uma leitura escalar como recurso metodológico para explicação da realidade com o propósito de compreender que, embora haja uma valorização da instância local para o desenvolvimento local, a escala global é definidora das políticas públicas pensadas para o município. Através do método dialético e da pesquisa qualitativa, os sujeitos sociais foram ouvidos e suas falas interpretadas. A análise da política de descentralização da saúde considerou a redefinição do papel do Estado no contexto da mundialização do capital que pratica novo ordenamento territorial, centrado no município, chamado para dar respostas à crise financeira, fiscal e melhorar a capacidade de gestão do Estado. A pesquisa desenvolvida em Sergipe constatou que a política de descentralização da saúde mascara a apropriação privada dos serviços e ações de saúde no âmbito local/nacional/global através de uma rede seletiva e excludente. Para isso concorre o tratamento patrimonial da coisa pública que alimenta a cultura do favor e desencadeia disputas políticas descomprometidas com o atendimento das necessidades de saúde da população. _______________________________________________________________________________________ ABSTRACT: The central concern of this article is to analyze the decentralization politics of health that created SUS and with it, a redistribution of political power, strengthening the municipal district for operalization of the politics of health. It is started from a scalar reading as methodological resource for explanation of the reality with the purpose of understanding that, although there is a valorization of the local instance for the local development, the global scale is definer of the public politics thought for the municipal district. Through the dialectical method and of the qualitative research, the social subjects were heard and their speeches were interpreted. The analysis of the ecentralization politics of health considered the redefinition of the role of the State in the context of the globalization of the capital that practices new territorial ordainment, centered in the municipal district, called to give answers to the financial crisis, fiscal and to improve the capacity of administration of the State. The research developed in the state of Sergipe verified that the decentralization politics of health masks the deprived appropriation of the services and actions of health in the local/national/global ambit through a selective and excluding net. For that, it competes the patrimonial treatment of the public thing that feeds the culture of the favor and unchains uncommitted political disputes with the attendance of the population needs of health.
