Amelogênese imperfeita e reabsorção radicular inflamatória externa: relato de caso

Resumo: Patients with Amelogenesis Imperfect (AI) may be more susceptible to dental caries due to the greater fragility and irregularity of the dental enamel, thus favoring the accumulation of bacterial biofilm, and consequently the formation of caries disease and aggression to the pulp tissue. This study aimed to describe a clinical case of endodontic treatment in a patient with amelogenesis imperfect and external inflammatory root resorption in the lower first permanent molars. A 22-years-old male patient was diagnosed with Hypocalcified Amelogenesis Imperfect. Clinically his lower first permanent molars had extensive temporary restoration associated with negative response to thermal, percussion and palpation tests. The periapical and panoramic radiographic images demonstrated the presence of a well-defined radiolucent image associated with both right and left first molars apex, as well as external root resorption in the apical region of the distal root of these molars. The treatment plan involved pulpectomy of the lower first molars that was perfomed with the use of calcium hydroxide as intracanal medication for 14 days. After root canals filling, definitive restoration was performed and both teeth were followed-up during 6 months, demonstrating absence of symptoms, as well as stabilization of root resorption and repair of the periapical region for the right first molar. On the other hand, the left first molar did not show regression of periapical radiolucency or stabilization of root resorption and periapical surgery was planned. The conventional endodontic approach to repair periapical lesion and external root resorption in the patient with Amelogenesis Imperfect was a successful treatment only for one of the lower molars. Facing these challenges, the professional must be prepared to present treatment alternatives in an attempt to prevent tooth loss, as well as work together with other areas to provide the best treatment for the patient.

Este trabalho tem como objetivo descrever um caso clínico de tratamento endodôntico em paciente com amelogênese imperfeita e reabsorção radicular inflamatória externa nos seus primeiros molares inferiores permanentes. Paciente do gênero masculino com 22 anos de idade foi diagnosticado com Amelogênese Imperfeita do tipo hipocalcificada. Clinicamente seus primeiros molares inferiores permanentes apresentavam restauração provisória extensa, ausência de resposta aos testes térmico, de percussão e palpação. O exame radiográfico periapical e panorâmico demonstraram a presença de imagem radiolucida apical, circunscrita, e bem definida associada às raízes dos primeiros molares inferiores direito e esquerdo, bem como reabsorção radicular externa na região apical dos referidos molares. O plano de tratamento envolveu a pulpectomia dos dois molares que foi realizada em sessões intercaladas pelo uso de medicação intracanal à base de hidróxido de cálcio por 14 dias. Após a obturação dos canais radiculares e restauração definitiva, os dentes foram acompanhados por até 6 meses, sendo observada ausência de sintomatologia para ambos os dentes, bem como paralisação da reabsorção radicular e reparo da região periapical para o primeiro molar direito. Por outro lado, o primeiro molar esquerdo não demonstrou regressão da radioluscência periapical nem paralisação da reabsorção, sendo planejada cirurgia periapical do mesmo. O tratamento endodôntico convencional para o comprometimento periapical e reabsorção radicular externa no paciente com Amelogênese Imperfeita obteve sucesso apenas em um dos molares inferiores.
