Avaliação da qualidade de vida relacionada à saúde em indivíduos com fissura labial e/ou palatina e seus familiares

Resumo: Introduction: The nonsyndromic orofacial cleft is the fourth birth defect and the most common craniofacial malformation. Problems such as anxiety, depression, difficulty in coping with the malformation diagnosis and meet the special needs of these children are quite common, affecting thereby the quality of life of the fissured individuals and their families. Objective: The aim of this study was to evaluate the Health Related Quality of Life (HRQoL) in individuals with cleft lip and/or palate (CLP) and their relatives. Methodology: It was performed a descriptive, observational, cross-sectional, in the largest referral facility for service individuals with cleft in Sergipe state. For this research, we selected three groups, with 97 subjects each: Individuals with cleft, Family and Control. Initially, the cognitive evaluation was performed with the Mini-Mental State Examination, and in individuals with sufficient cognitive capacity, It was applied the MOS 36 item Short-Form Health Survey (SF-36), to evaluate HRQoL. The SF-36 was delivered to the cleft individuals, their relatives and to the control group. It was self-administered to the individuals with formal education. For the five individuals that were illiterate the SF-36 was applied as an interview. Results: It was observed in this study that individuals with CLP, mostly, had primary education, were single, with incomes between one and two minimum wages and little more than a half of these were individuals with transforamen cleft (59.8%). Moreover, there was no significant difference in the overall HRQoL score in the three groups (cleft: 72.2; family: 70.6; control: 72.5). However, when It was analyzed the SF-36 domains, some significant differences were found. In the vitality domain, individuals with cleft had higher average than the control and the family groups. In the emotional domain, the control group showed a greater mean that the individuals with cleft and that the family. The males had higher averages in the physical function and mental health domains. Individuals operated in optimal time (≤ 12 months) had higher mean in the physical function domain. Conclusion: We conclude that the Health Related Quality of Life was similar among individuals with CLP, their family and the population control. But when analyzing the SF-36 domains, it was observed that the family and the CLP individuals had lower mean in the emotional aspects domain when compared with the control population. Furthermore, patients with cleft lip and / or palate females had a lower HRQoL.

Problemas como ansiedade, depressão, dificuldade em lidar com o diagnóstico da malformação e suprir as necessidades especiais dessa criança são muito comuns, afetando, dessa forma, a qualidade de vida dos indivíduos fissurados e de seus familiares. Objetivo: o objetivo deste estudo foi avaliar a Qualidade de Vida Relacionada à Saúde (QVRS) em indivíduos com fissura labial e/ou palatina (FLP) e seus familiares. Casuística e métodos: foi realizado um estudo descritivo, observacional, transversal, no maior serviço de referência de atendimento ao fissurado do Estado de Sergipe. Para esta pesquisa, foram selecionados 3 grupos, com 97 indivíduos cada um: Fissurados, Familiares e Controle. Inicialmente, foi realizada a avaliação cognitiva através do Mini-Exame do Estado Mental e, nos indivíduos com capacidade cognitiva suficiente, foi aplicado o MOS 36 item Short-Form Health Survey (SF-36), para avaliação da QVRS. O SF-36 foi entregue aos fissurados, aos familiares e ao grupo controle, sendo autoadministrado de forma aberta e presencial para os indivíduos com educação formal prévia. Para os cinco indivíduos analfabetos a aplicação do questionário SF-36 se deu em forma de entrevista. Em relação ao sexo, não houve discrepância na distribuição entre homens e mulheres para os grupos fissurados e controle. Não foi observada diferença significativa quanto ao escore geral de QVRS nos três grupos (fissurados: 72,2; familiares: 70,6; controle: 72,5). Entretanto, quando foram analisados os domínios do questionário SF-36, algumas diferenças significativas foram encontradas. No domínio vitalidade, os indivíduos com fissura obtiveram maior média que o controle e que os familiares. Quando ao domínio aspectos emocionais, o grupo controle apresentou maior média que os indivíduos com fissura e que os familiares. O sexo masculino apresentou maiores médias para os domínios capacidade funcional e saúde mental. Indivíduos operados no tempo ideal (≤12 meses idade) obtiveram maior média quanto à capacidade funcional. Não foi observada diferença significativa para a QVRS entre os diferentes tipos de fissura (pré, pós e transforame. Conclusão: a Qualidade de Vida Relacionada à Saúde global foi semelhante entre os indivíduos com FLP, seus familiares e a população controle.
