Introduction: The posture is defined as a state of corporal alignment, and it is influenced by several intrinsic factors, such as joint positioning, trophism, muscle strength and length, and extrinsic as repetitive stress and the individual's positioning. In children, the appearance of postural changes is due to factors related to child growth and development as well as environmental factors. Objectives: Relate the posture of school age from 7 to 10 years with the variables weight of the backpack, pain, and body mass index. Methods: 115 students were enrolled in municipal and state public elementary schools in the city of Lagarto / SE, both males and females, aged between seven and ten years. The following variables were evaluated: body weight, height, body mass index (BMI), backpack weight, posture and pain. Postural evaluation was performed using computerized photogrammetry using Corel Draw Graphics Suite 2017 software. They were appraised angle of head and shoulder and the distances intercondilar (ICD) and intermalleolar (IMD), for these last ones a measuring tape was used. The pain was evaluated using the numeric scale of the pain. Results: In the correlation analysis among the variables relationship was observed between weight of the backpack and ICD (p<001), head angle (p=003) and shoulder angle (p<001) and the relationship of BMI with ICD (p=004), IMD (p<001), angle of head (p<001) and shoulder angle (p=002), there was not significant relationship between the intensity of the pain and the posture variables. Conclusion: BMI and inadequate backpack weight may be predisposing factors to postural changes in children.
