Representações do trabalho médico em saúde da família : algumas considerações sobre Brasil e Portugal

Resumo: The world of work , as well as the world of training , have different specificities that are both paradoxical as complementary and influence the process of socialization. In view of this , this study aimed to analyze the representations and experiences of physicians who work in teams of the Family Health in Brazil / Portugal and Aracaju . Special emphasis was given to the construction of their careers and identities before the new qualification requirements and demands for new skills . Given the nature of the object, we adopted a qualitative approach inspired historical- dialectical , considering the macro-micro , individual - collective relationship with its conflicts and contradictions in multicase study . It is understood that the forms of sociability differentiate and are marked by contradictions and specificity , in that direction , the subject , as individual and collective actors engaged in certain organizational and institutional contexts both create social relations as they are also a product of these . For this purpose we used different information sources . The research literature has discussed the methodological, conceptual inputs of qualification policy and labor ; documental analysis were elected and coded discourses related to the work context of medical FHS and USF , its organizational structure and relationship with the qualification requirements present on documents in both realities studied . Systematic observation was made in the development of training courses offered to medical professionals who perform activities in the FHS in Brazil / Aracaju , and USF / Portugal , through the Center for Continuing Education in Health ( CEPS ) and the National Association of Family Health Units (USF -AN ) . The semistructured interview was privileged to capture the social representations of 24 medical research participants. The results demonstrate the detachment and the fragility of the model of the initial training of doctors based on technical rationality, decontextualized to meet the demands of working professionals who work under the prism of Primary Health Care Passage of a technical representation of the family doctor representation to a more interdisciplinary, with a strong emphasis on social , is a trend in both realities . In both contexts studied, this dimension is shown marked by conflict , denial - process claim against the knowledge-power of the physician. This professional is constructed as a social and individual subject , the relationship with colleagues and users . Therefore, one can not speak of medical identity, meaning and significance without considering the context of action prints in its conformation . For the subjects , work means a privileged social production of identities locus . At the same time , in the case of Brazil / Sergipe / Aracaju , there is a need for a more effective approach of the training process offered by official bodies . Realized the need for effective negotiation space between managers and professionals FHS teams. The policy of education developed at the local level is shown fragmented, adequacy or directed to achieving a new demand within the labor process, ie , to the prescribed work . However , it is concluded that , despite the difficulties, this policy has produced partial changes in practices , social relations , the socialization processes of these subjects even those not aware of it .

Em vista disso, este estudo objetivou analisar as representações e experiências de médicos que atuam nas equipes de Saúde da Família no Brasil/Aracaju e em Portugal. Especial destaque foi atribuído à construção de suas trajetórias e identidades diante dos novos requisitos de qualificação e de demandas por novas competências. Dada à natureza do objeto, adotou-se a abordagem qualitativa com inspiração histórico-dialética, considerando-se a relação macro-micro, coletivo-individual, com seus conflitos e contradições no estudo de multicasos. Compreende-se que as formas de sociabilidades diferenciam-se e são marcadas por especificidade e contradições; nessa direção, os sujeitos, como atores individuais e coletivos inseridos em determinados contextos organizacionais e institucionais, tanto criam relações sociais como também são produto destas. Para tanto utilizaram-se diferentes fontes de informação. A investigação bibliográfica discutiu os aportes metodológicos, conceituais da política de qualificação e do trabalho; na análise documental foram eleitos e codificados os discursos referentes ao contexto de trabalho dos médicos da ESF e da USF, a sua estrutura organizacional e a relação com os requisitos de qualificação presentes nos documentos nas duas realidades estudadas. A observação sistemática foi realizada no desenvolvimento dos cursos de qualificação ofertados para os profissionais médicos que exercem suas atividades na ESF, no Brasil/Aracaju, e na USF/Portugal, através do Centro de Educação Permanente em Saúde (CEPS) e da Associação Nacional das Unidades de Saúde Familiar (USF-AN). Foi privilegiada a entrevista semiestruturada para captar as representações sociais de 24 médicos participantes da pesquisa. Os resultados demonstram o distanciamento e a fragilidade do modelo da formação inicial dos médicos, baseado na racionalidade técnica, descontextualizada para atender as demandas de trabalho dos profissionais que atuam sob o prisma da Atenção Primária à Saúde. A passagem de uma representação tecnicista do Médico de Família para uma representação mais interdisciplinar, com forte ênfase no social, é uma tendência observada nas duas realidades. Nos dois contextos estudados, tal dimensão mostra-se marcada por conflitos, processo de negação-afirmação frente ao saber-poder do médico. Esse profissional constrói-se enquanto um sujeito social e singular, na relação com os colegas e com os usuários. Portanto, não se pode falar da identidade médica, sem considerar sentido e o significado que o seu contexto de ação imprime na sua conformação. Para os sujeitos da pesquisa, o trabalho significa um locus privilegiado de produção social de identidades. Ao mesmo tempo, no caso do Brasil/Sergipe/Aracaju, observa-se a necessidade de uma aproximação mais efetiva dos processos de qualificação ofertados pelos organismos oficiais. Percebeu-se a necessidade de espaço de negociação efetiva entre gestores e profissionais das equipes da ESF. A política de educação permanente desenvolvida em nível local mostra-se fragmentada, direcionada para adequação ou consecução de uma nova demanda no interior do processo de trabalho, ou seja, para o trabalho prescrito.
