Comparação do padrão citológico de incidentalomas em relação a nódulos tireoidianos clinicamente relevantes

Resumo: Introduction: The clinical spectrum of thyroid nodules vary from an incidental and asymptomatic nodule, in 59 which cancer is the major concern, to clinically relevant nodules (= 1 cm) with compressive symptoms. This 60 study aims to compare clinical, ultrasonographic and cytological features of thyroid incidentalomas with 61 Materials and Methods: This cross-sectional retrospective study included 812 patients with thyroid 62 nodules, who underwent thyroid ultrasonography and Fine Needle Aspiration from January 2007 to 63 December 2013. We collected clinical and sonographic characteristics, and cytology was expressed 64 according to the Bethesda classification. Results: Of the 812 patients, 32.1% presented incidentalomas (< 1 65 cm). Thyroid incidentalomas presented as solid hypoechogenic and with peripheral vascularization lesions, 66 while nodules = 1 cm were mixed, hyperechogenic, with central and peripheral vascularization. Bethesda I 67 class was most commonly found among incidentalomas, and Bethesda II in the clinically relevant nodules’ 68 group. The frequency of malignant cytologies (Bethesda V and VI) was low in both groups. Conclusion: 69 Despite the ultrasound characteristics found in thyroid incidentalomas, they did not present a higher 70 frequency of suspicious or malignant cytologies. Instead, insufficient results were more frequent in this 71 group, which reinforces the current tendency to value suspicious sonographic characteristics altogether and 72 consider the possibility of follow-up of subcentimetric nodules.

O objetivo deste estudo foi comparar as características clínicas, ultrassonográficas e 32 citológicas dos incidentalomas tireoidianos com os nódulos clinicamente relevantes (= 1 cm). Materiais e 33 Métodos: Estudo transversal e retrospectivo, incluiu 812 pacientes portadores de nódulos tireoidianos 34 submetidos à ultrassonografia da tireoide e Punção Aspirativa por Agulha Fina, no período de Janeiro de 35 2007 a Dezembro de 2013. Foram levantadas características clínicas e ultrassonográficas. A citologia foi 36 expressa de acordo com a classificação Bethesda. Resultados: Dos 812 pacientes, 32,1% apresentaram 37 incidentalomas (< 1 cm). Os incidentalomas apresentaram-se como nódulos sólidos, hipoecogênicos e com 38 vascularização periférica, enquanto os nódulos = 1 cm eram mais frequentemente mistos, hiperecogênicos, 39 com vascularização central e periférica. A classe Bethesda I foi mais encontrada no grupo dos 40 incidentalomas, enquanto a Bethesda II no dos clinicamente relevantes. A frequência de citologias malignas 41 (Bethesda V e VI) foi baixa em ambos os grupos.
