Resumo: Perinatal mortality is still one of the biggest problems of Brazilian public health and one of the most considered health indicators. The description of perinatal deaths in a high-risk maternity hospital aims to define which problems still affect this group. Thus, a retrospective study was conducted in a high-risk maternity hospital, identifying 260 early fetal and neonatal deaths that occurred within a 12-month period - between January and December 2018. Through review of medical records, relevant data were collected since pregnancy. until the moment of death, as well as the factors that triggered it. The data were also analyzed based on the correction of the death certificates issued by the death committee of the maternity ward itself. The perinatal mortality rate was 46.5 per 1,000 live births, more than double the state rate in the year prior to the study. The profile of deaths presented characteristics that deserve better attention from public policies in search of the avoidability of these problems, such as a high incidence of extreme prematurity (23.5%), a good reduction due to better attention to the health of high-risk pregnant women. In addition, there is still a high number of perinatal deaths of fetuses and newborns greater than 1500g (40.8%) and term (19.2%), and the dead fetuses make up 2/3 of this population and had their deaths associated with maternal conditions without adequate intervention.

A descrição dos óbitos perinatais em uma maternidade de alto risco objetiva definir quais os problemas ainda afetam este grupo. Desse modo, foi realizado um estudo retrospectivo em maternidade de alto risco, identificando-se 260 óbitos fetais e neonatais precoces ocorridos num período de 12 meses - entre janeiro e dezembro de 2018. Por meio de revisão de prontuário foram colhidos dados relevantes desde a gestação até o momento do óbito, bem como os fatores que o desencadearam. Os dados também foram analisados a partir da correção das declarações de óbito dispostas pela comissão de óbito da própria maternidade. A taxa de mortalidade perinatal foi de 46,5 por mil nascidos vivos, mais que o dobro da taxa estadual no ano anterior ao estudo. O perfil dos óbitos apresentou características que merecem uma melhor atenção das políticas públicas em busca da evitabilidade desses problemas, como alta incidência de prematuridade extrema (23,5%), boa parte reduzível por melhor atenção à saúde da gestante de alto risco.
