Conduta de ginecologistas frente a ectopia cervical

Resumo: Cervical ectopia uterine is a physiological phenomenon where there is eversion of the endocervical mucosa to the ectocervix. After its development, the exposed simple columnar epithelium undergoes a process of metaplasia, and may take months or years to recover. Prolonged exposure of regions of metaplasia or simple columnar epithelium in the ectocervix leaves the cervix more susceptible to carcinogenic lesions and recurrent infections by sexually transmitted microorganisms. Objective: To identify the conduct of physicians, who act as gynecologists in the Northeast, regarding cervical ectopy and to evaluate if there is an association between the adopted conduct and sociodemographic aspects and training. Method: the study was conducted during the XXX Nordestine Congress of Gynecology and Obstetrics / 45th Pernambuco Congress of Gynecology and Obstetrics, in June 2019. A semistructured interview was applied to 131 physicians working in the gynecology area, with sociodemographic questions from training and the conduct adopted. Results: most gynecologists opted for expectant management (57.3%); followed by cauterization, chemical or electrical, if symptoms (37.4%). The expectant approach was more adopted by young people and those over 56 showed preference for cauterization (p = 0.049). Conclusion: There was a predominance of expectant or interventionist conduct in the presence of symptoms. Prophylactic routine cauterization is not often adopted

Após seu desenvolvimento, o epitélio colunar simples exposto sofre um processo de metaplasia, podendo levar meses ou anos para se restabelecer. A exposição por um tempo prolongado de regiões de metaplasia ou de epitélio colunar simples na ectocérvice deixam o colo uterino mais suscetível a lesões carcinogênicas e infecções recorrentes por microrganismos sexualmente transmissíveis. Objetivo: identificar a conduta dos médicos, que atuam como ginecologistas na Região Nordeste, perante a ectopia cervical e avaliar se há associação entre conduta adotada e aspectos sociodemográficos e formação. Método: o estudo foi realizado durante o XXX Congresso Nordestino de Ginecologia e Obstetrícia/45º Congresso Pernambucano de Ginecologia e Obstetrícia, em junho de 2019. Aplicou-se uma entrevista semiestruturada a 131 médicos que atuam na área de ginecologia, com questões sociodemográficas, de formação e a respeito da conduta adotada. Resultados: a maior parte dos ginecologistas optaram pela conduta expectante (57,3 %); seguida pela cauterização, química ou elétrica, se sintomas (37,4 %). A conduta expectante foi mais adotada por jovens e os com idade superior a 56 demonstraram preferência pela cauterização (p=0,049). Conclusão: Houve predomínio da conduta expectante ou intervencionista na presença de sintomas.
