Resumo: The main component of infant mortality today is the early neonatal period. Neonatal near miss is defined as a newborn who presented a severe complication during the first days of life, but survived the neonatal period. As there are more cases of neonatal near miss than neonatal deaths, a better evaluation of maternal and neonatal care is possible through its study. This study aims to describe the maternal-infant characteristics of neonatal near miss cases in a high-risk maternity hospital in Aracaju, Sergipe, Brazil. It is a cross-sectional study nested in a case control, performed at a high-risk maternity, from March to September 2015. The criteria for classification in neonatal near miss were gestational age less than 32 weeks, birth weight less than 1500g and/or Apgar score at 5 minutes less than 7. All cases of neonatal near miss in this period were identified and a form was filled out for each of them, with information on the pregnant card, medical chart and through an interview with the mother. A total of 3270 newborns were analyzed, of which 125 were neonatal near miss cases. The mean maternal age was 25.9 years, 23.2% of the mothers lived in Aracaju, 58.4% did not work, 88.8% had obstetric complications, 48.8% underwent cesarean sections, 8.8% had no prenatal care and 48% had less than six prenatal visits. Among neonates classified as neonatal near miss, 50.4% were males, 69.6% had a birth weight less than 1500g, 52.8% had gestational age less than 32 weeks and 24.8% presented Apgar in the fifth minute less than 7. The profile found in the cases reveals newborns, mostly males, of gestational age less than 32 weeks, very low birth weight, born of normal birth, with mothers born in Aracaju, who had less than 6 prenatal visits or had no prenatal care, had complications during pregnancy, did not work and had low educational level. The characteristics found in this research are useful for directing the financing of public health policies aimed at improving the quality of maternal and child care.
