Resumo: Backgroud: Transsexuality is a relatively new topic in the scientific publications. Transgender patients require different kinds of therapy for suitability of your phenotype to the gender for which they feel belonging. As for those transgender men, people that were born on female gender, but have a male gender performance, the use of testosterone means bodily aesthetics and metabolic changes. Purpose: Describe cytopathological characteristics from Pap tests in transgender men submitted to hormonal treatment with testosterone in a reference outpatient. Methods: This is a cross-sectional descriptive study with evaluation of cytological findings: research of epithelial changes precursors of uterine cervix cancer, identification of microbiological flora most common in users and description of possible changes elapsed of the chronic use of androgen hormones. Results: No neoplasia evidences were found in this study. Lactobacillus were the germs more prevalent and inflammatory findings in 93,6% of the examined slides were observed. Cellular changes on the chromatin which were described to look granular and thin in shape, distributed regularly, besides an increase of the relation nucleus/cytoplasm with hyperchromasia of nucleus. Conclusions: The reactive findings of inflammation and atrophy were of high incidence of population in the study, therefore, thinking that testosterone is capable of cause such procedures even in a short period of application. It is necessary promote longitudinal studies, that monitor these findings with deadlines use of longer androgenic hormones.
