Resumo: Introduction: Human Chagas disease, also known as American trypanosomiasis, is a global health issue. About seven million people around the world are infected, according to World Health Organization (WHO). The vector is one of the main transmission routes in Brazil, South and Central American countries. The State of Sergipe has shown a high prevalence of T. cruzi infested triatomines captured between 2013 and 2019, with the villages of Poço da Clara, Alagoinhas and Pilões, in the town of Tobias Barreto, showing the highest registers of the infestation, motivating the epidemiological study. Methods: A cross-sectional study with a quantitative approach was conducted, divided into three stages: the first verified the infection in humans through serological analysis for Trypanossoma cruzi, by at least two laboratory methods, the application of a questionnaire with demographic data and risk variables for the disease. The second stage, after the identification of the risk variables for the transmission of the infection, vectors were searched and captured in the households for entomological and parasitological analysis by LACEN – Sergipe State Central Laboratory. The third stage consisted of the serological evaluation in domestic reservoirs found in the households and peridomiciliary areas.The results were stored in a database in the Excel program, version contained in the Microsoft Office 365 license, analyzed by Epi Info version 7.1.4 and Ambiente R version 3.6.3. The analysis of the variables was performed using the Pearson 's Chi - square test, Monte Carlo simulations were used in the presence of expected frequencies less than 5, and Yates' correction in the opposite case. The analysis of the spatial autocorrelation of the triatomines in the study was performed with GeoDa software version 1.14, using the univariate global Moran index and pseudo-significance test for non-normal distributions.Statistical tests were performed at the 5% probability level, in other words, 95% trust level. Results: Serological evaluation of the population identified 0.24% of T. cruzi infection in humans; serologies from domestic reservoirs were also evaluated. The species Triatoma pseudomaculata, Pantronsgylus lutzi, and Triatoma tibiamaculata were identified in the study sites with higher intradomicilium prevalence. Conclusion: There is chronic human infection of Chagas disease in rural area of Tobias Barreto, south of Sergipe state, in the village of Alagoinhas. There is vector transmission in the studied locations, and the risk is maintained. The results confirm that the disease is not being diagnosed and that vector transmission is not being interrupted, expressing the need for changes in prevention and control policies.

A via de transmissão vetorial é uma das mais importantes nos países da América do Sul e Central; no Brasil a via oral vem assumindo importante papel na transmissão, seguida da via vetorial em áreas com a presença de triatomíneos. O estado de Sergipe apresentou alta prevalência com cerca de 507 triatomíneos analisados entre os anos de 2013 a 2019; os maiores registros de infestação foram nos povoados de Poço da Clara, Alagoinhas e Pilões, município de Tobias Barreto, motivando o estudo epidemiológico. Métodos: Foi realizado um estudo transversal com abordagem quantitativa, dividido em três etapas: a primeira verificou a infecção em humanos por meio da análise sorológica de 419 indivíduos para Trypanossoma cruzi, por pelo menos dois métodos laboratoriais, aplicação de questionário para coleta de dados socioeconômicos e demográficos, e variáveis de risco para transmissão da doença. Na segunda, após identificação das variáveis foi realizado busca e captura de vetores, análise entomológica e parasitológica das espécies prevalentes nos locais do estudo, e coleta de dados das espécies de triatomíneos analisadas pelo Laboratório Central de Saúde Pública de Sergipe. A terceira etapa consistiu na avaliação sorológica de reservatórios domésticos, encontrados nos domicílios e áreas peridomiciliares nas quais foram capturados triatomíneos infestados. Os resultados foram armazenados em banco de dados no programa Excel, versão contida na licença de Microsoft Office 365, analisados pelo programa Epi Info versão 7.1.4 e Ambiente R versão 3.6.3. A análise das variáveis foi realizada através do teste Qui – quadrado de Pearson, foram utilizadas simulações de Monte Carlo na presença de frequências esperadas menores que 5, e correção de Yates em caso contrário. Os testes estatísticos foram realizados com o nível de 5% de probabilidade, ou seja, 95% de confiança. A análise da autocorrelação espacial dos triatomíneos no estudo foi realizada com software GeoDa versão1.14, utilizando o índice de Moran global univariado e teste de pseudo-significância para distribuições não normais. Os testes estatísticos foram realizados com o nível de 5% de probabilidade, ou seja, 95% de confiança. Resultados: A avaliação sorológica da população identificou 0,24% de infecção por T. cruzi em humanos. Em Sergipe 83,03% dos triatomíneos foram encontrados no intradomicílio, a taxa de infecção natural no estado foi de 7,49%. Em Sergipe e nos povoados do estudo houve predomínio das espécies P. lutzi e T. pseudomaculata. As sorologias dos reservatórios domésticos foram negativas. Conclusão: Há infecção humana crônica da doença de Chagas na área rural de Tobias Barreto, sul do estado de Sergipe. Existe transmissão vetorial, embora o percentual conhecido de infecção em humanos na área foi inferior à estimativa de prevalência nacional demonstrando haver subnotificação. A convivência com animais é um fator de risco relevante para transmissão.
