Determinantes da distância percorrida no teste de caminhada de seis minutos no momento da alta hospitalar de pacientes com insuficiência cardíaca

Introduction: The six-minute walk test (6MWT) have been considered a benchmark in the assessment of the functional capacity of several populations. The distance walking in this test is used as a measure to assess the risk of hospitalization, mortality, and the functional prognosis of cardiac patients. Objective: To identify the factors that determine the distance covered in the six-minute walk test (6MWD) at the time of hospital discharge from patients with heart failure and to establish a reference equation for calculating the predicted 6MWT.. Materials and Methods: This is a descriptive and cross-sectional clinical study. The sample consisted of patients with heart failure functional class I-III. Data were collected: age; sex; BMI (body mass index); left ventricular ejection fraction (LVEF); heart rate delta (between the initial and final of the 6MWT), hemoglobin; fasting blood glucose; etiology of the disease; time of diagnosis; length of hospital stay; presence of risk factors for cardiovascular diseases and presence of infection during hospitalization. The muscle strength of the knee extension movement was assessed by dynamometry, quality of life with the Nottingham Health Profile (NHP) questionnaire, the previous physical activity level by the International Physical Activity Questionnaire (IPAQ) and the 6MWT. Results: Sixty patients were part of the final sample, 58.3% male; with a mean age of 56.23 ± 10.44 years. The mean of the 6MWD was 393.16 ± 42.80 meters. The variables selected from the multivariate analysis were: IPAQ (p = 0.017), mean maximum isometric muscle strength (p = 0.001), total NHP (p = 0.021) and age (p = 0.046). R = 0.74 and R² = 0.55 with p <0.05. Conclusion: In this study, the determinants of 6MWD were: IPAQ, maximal isometric knee extension force, total PSN and age. It was possible with this study to generate a predictive equation for the 6MWT at discharge from patients with heart failure.
