This study had as object the difficulties of the family participation in the treatment of users of the services of Psychosocial Care Centers in Brazil. It is a bibliographical research, whose data collection was done from surveys of materials already published as articles, dissertations, theses, books and other types of publications by Scielo and Google. Its objective was to signal the difficulties experienced by these families and their expressions described in the literature found about them. Several types of difficulties faced by family members with regard to care of the mentally ill person could be identified: the gap between the rhythm of life of the patient and that of the family / society; conflicts within the family; guilt / acceptance difficulties of the disease; difficulty in dealing with the real and the desire not to live the situation; lack of management in dealing with the symptomatology resulting from the mental disorder; prejudice with the problem of the "madness" existing in society and difficulties with the Health Service. It is concluded that despite all these difficulties that families face with mental suffering in their context, it is a very important support in the care of these people's health and inclusion in social life.
