Construção de subconjunto terminológico da CIPE® para pacientes com lúpus eritematoso sistêmico

Resumo: It is a study that, in order to meet the proposed objectives, used two study designs, an integrative review and a methodological study with a quantitative approach that aimed to elaborate a Terminology Subset of the International Classification for Nursing Practice - ICNP® for patients with Systemic Lupus Erythematosus (SLE). To base the study was used as theoretical reference the Basic Human Needs Theory. For the review study, the sample consisted of articles on nursing care for SLE patients published in the last 10 years in the english, spanish and portuguese languages, and the following descriptors and connectives were used: lúpus eritematoso sistêmico, systemic lupus erythematosus, lupus eritematoso sistémico and/or; cuidados de enfermagem, nursing care, atención de enfermería and/or; avaliação em enfermagem, nursing assessment, evaluación en enfermería and/or. For the methodological design, the samples were composed of five and six judges who validated the terms extracted from the articles and the statements of diagnosis/results/nursing interventions for SLE patients, respectively. The study was carried out through the following steps: integrative review to obtain articles on nursing care for SLE patients; extraction of terms relevant to nursing practice related to SLE patients with subsequent validation of these terms; cross-mapping of validated terms with the terms of the Seven-Axis Model of ICNP® 2017; elaboration of diagnostic statements/results and nursing interventions for SLE patients; followed by the cross-mapping of the statements and their validation; distribution of statements according to the Theory of Basic Human Needs; and elaboration of the ICPN® terminology subset for SLE patients. The integrative review to identify the terms resulted in six articles selected in the LILACS and MEDLINE databases. After the process of extracting the terms of these articles and normalizing the terms, a list with 831 terms was generated, validating 794, which were mapped and crossed with those in ICPN® 2017, evidencing 376 constant terms and 418 non constant terms classification. Thus, the special language terms database for SLE patients was used, which was used in the elaboration of 129 diagnostic statements/results and 253 statements of interventions, which were mapped and crossed with the pre-coordinates of diagnoses/results and nursing interventions of ICPN® 2017, resulting in 76 constant statements and 53 non constant statements of diagnosis/results, and 17 constant statements and 236 non constant statements of nursing interventions. After that, these statements were submitted to the validation process, which resulted in 104 diagnoses/results statements and 240 statements of validated interventions, which were distributed according to the theoretical framework in 14 psychobiological needs, nine psychosocial needs and one psychospiritual, being, in this way, the terminological subset for patients with SLE. It is hoped that this subset can be configured as an instrument to facilitate the implementation of a systematized and individualized approach to the SLE patient.________________________________________________________________________________________________________________________________

Para fundamentar el estudio fue utilizado como referencial teórico la Teoría de las Necesidades Humanas Básicas. Para el estudio de revisión muestra está formada por elementos que se encontraban sobre el cuidado de enfermería para los pacientes con LES, publicados en los últimos 10 años, en inglés, español y portugués, y utilizó los siguientes descriptores y conectivo: lúpus eritematoso sistêmico, systemic lupus erythematosus, lupus eritematoso sistémico and/or; cuidados de enfermagem, nursing care, atención de enfermería and/or; avaliação em enfermagem, nursing assessment, evaluación en enfermería and/or. Para el diseño metodológico las muestras fueron compuestas por cinco y seis jueces que validaron los términos extraídos de los artículos y los enunciados de diagnósticos/resultados/ intervenciones de enfermería para pacientes con LES, respectivamente. El estudio fue realizado por medio de las siguientes etapas: revisión integrativa para la obtención de artículos sobre asistencia de enfermería a pacientes con LES; extracción de los términos relevantes para la práctica de enfermería relacionada a los pacientes con LES con posterior validación de esos términos; mapeo cruzado de los términos validados con los términos del Modelo de Siete Ejes de la CIPE® 2017; elaboración de los enunciados de diagnósticos/resultados e intervenciones de enfermería para pacientes con LES; seguido del mapeo cruzado de los enunciados y de la validación de los mismos; distribución de los enunciados de acuerdo con la Teoría de las Necesidades Humanas Básicas; y la elaboración del subconjunto terminológico de la CIPE® para pacientes con LES. La revisión integrativa para la identificación de los términos resultó en seis artículos seleccionados en las bases de datos LILACS y MEDLINE. Después de realizado el proceso de extracción de los términos de estos artículos y normalización de los términos se generó una lista con 831 términos, siendo validados 794, los cuales fueron mapeados y cruzados con los existentes en la CIPE® 2017, evidenciando 376 términos constantes y 418 términos no constantes en esa clasificación. De esta forma, se constituye el banco de términos del lenguaje especial de enfermería para pacientes con LES, el cual fue utilizado en la elaboración de 129 enunciados de diagnósticos/resultados y 253 enunciados de intervenciones, que fueron mapeados y cruzados con los conceptos pre-coordinados de diagnósticos/resultados e intervenciones de enfermería de la CIPE® 2017, resultando en 76 enunciados constantes y 53 enunciados no constantes de diagnósticos/resultados, y en 17 enunciados constantes y 236 enunciados no constantes de intervenciones de enfermería. Después de eso, estos enunciados fueron sometidos al proceso de validación, que resultó en 104 enunciados de diagnósticos/resultados y 240 enunciados de intervenciones validadas, los cuales fueron distribuidos de acuerdo con el referencial teórico en 14 necesidades psicobiológicas, nueve necesidades psicosociales y una psicoespiritual, y de esta manera se organizó el subconjunto terminológico para pacientes con LES. Se espera que este subconjunto pueda configurarse como un instrumento facilitador para la implantación de un enfoque sistematizado e individualizado al paciente con LES.Trata-se de um estudo que para atender aos objetivos propostos utilizou dois desenhos de estudo, uma revisão integrativa e um estudo metodológico com abordagem quantitativa que teve como objetivo elaborar um Subconjunto Terminológico da Classificação Internacional para Prática de Enfermagem - CIPE® para pacientes com Lúpus Eritematoso Sistêmico (LES). Para o estudo de revisão a amostra foi composta por artigos que versaram sobre assistência de enfermagem a pacientes com LES, publicados nos últimos 10 anos, nos idiomas inglês, espanhol e português, e utilizados os seguintes descritores e conectivos: lúpus eritematoso sistêmico, systemic lupus erythematosus, lupus eritematoso sistémico and/or; cuidados de enfermagem, nursing care, atención de enfermería and/or; avaliação em enfermagem, nursing assessment, evaluación en enfermería and/or. Para o desenho metodológico as amostras foram compostas por cinco e seis juízes que validaram os termos extraídos dos artigos e os enunciados de diagnósticos/resultados/intervenções de enfermagem para pacientes com LES, respectivamente. O estudo foi realizado por meio das seguintes etapas: revisão integrativa para obtenção de artigos sobre assistência de enfermagem a pacientes com LES; extração dos termos relevantes para a prática de enfermagem relacionada aos pacientes com LES com posterior validação desses termos; mapeamento cruzado dos termos validados com os termos do Modelo de Sete Eixos da CIPE® 2017; elaboração dos enunciados de diagnósticos/resultados e intervenções de enfermagem para pacientes com LES; seguido do mapeamento cruzado dos enunciados e da validação dos mesmos; distribuição dos enunciados de acordo com a Teoria das Necessidades Humanas Básicas; e elaboração do subconjunto terminológico da CIPE® para pacientes com LES. A revisão integrativa para identificação dos termos resultou em seis artigos selecionados nas bases de dados LILACS e MEDLINE. Depois de realizado o processo de extração dos termos desses artigos e normalização dos termos foi gerada uma lista com 831 termos, sendo validados 794, os quais foram mapeados e cruzados com os existentes na CIPE® 2017, evidenciando 376 termos constantes e 418 termos não constantes nessa classificação. Após isso, esses enunciados foram submetidos ao processo de validação, que resultou em 104 enunciados de diagnósticos/resultados e 240 enunciados de intervenções validados, os quais foram distribuídos de acordo com o referencial teórico em 14 necessidades psicobiológicas, nove necessidades psicossociais e uma psicoespiritual, sendo, dessa maneira, organizado o subconjunto terminológico para pacientes com LES.
