Hypothermia for neonatal hypoxic ischemic encephalopathy using axillary temperature: an effective treatment in a low resource setting

Resumo: | Therapeutic hypothermia is the standard clinical practice for neonates with moderate to severe hypoxic ischaemic encephalopathy (HIE). Aim: To implement a therapeutic hypothermia protocol using axillary temperature in newborns with HIE, comparing the short term outcomes with a historical control group. Methods: Non-randomized clinical trial. Infants born at 36 weeks gestation with perinatal asphyxia were included in the study. An hypothermia protocol was applied in all neonates diagnosed with HIE admitted to the NICU. Results: Six newborns underwent therapeutic hypothermia and their clinical outcomes were compared with 18 newborns admitted previously (control group). Mortality was lower in hypothermia group as well as the presence of disabilities in hospital discharge. Conclusion: Therapeutic hypothermia for treatment of HIE in newborn infants seems to be a safe and effective procedure having a significant impact in reducing the mortality rate and improving the neurodevelopmental in short term. |

A hipotermia terapêutica é a prática clínica padrão para neonatos com encefalopatia isquêmica hipóxica moderada a grave (HIE). Objetivo: Implementar um protocolo de hipotermia terapêutica usando temperatura axilar em recém-nascidos com HIE, comparando os resultados a curto prazo com um grupo de controle histórico. Métodos: Ensaio clínico não randomizado. Os bebês nascidos com 36 semanas de gestação com asfixia perinatal foram incluídos no estudo. Um protocolo de hipotermia foi aplicado em todos os neonatos diagnosticados com HIE internados na UTIN. Resultados: Seis recém-nascidos foram submetidos a hipotermia terapêutica e seus resultados clínicos foram comparados com 18 recém-nascidos internados anteriormente (grupo controle). A mortalidade foi menor no grupo hipotermia e presença de incapacidades na alta hospitalar.
