Resumo: | Palliative care consists of a set of actions based on people with serious health-related suffering. In hospitals, the formation of qualified teams has been recognized as fundamental for planning, consulting healthcare professionals, as well as for providing integrated care to healthcare teams. To describe the implementation experiences and practices of Palliative Care teams in hospitals, described in the literature. Integrative review carried out in June 2021, through research of articles published in the Virtual Health Library, from January 2016 to March 2021, using as Descriptors: palliative care, multi-professional team, patient care team and hospital. The models highlighted by the studies are palliative care teams in a consultancy format; direct patient care at the request of the health team and; recently, in some countries, integrated into the system and acting autonomously. There is resistance in the implementation and performance of palliative care teams in hospitals, especially on the part of doctors, who also resist participating in the teams. However, the results achieved have contributed to breaking down barriers and promoting the evolution of these teams. There is a tendency to abandon exclusively consultative and/or on-demand models, to change them in to teams that work integrated and active at all levels of care. Em hospitais, a formação de equipes capacitadas tem sido reconhecida como fundamental para o planejamento, consultoria a profissionais de saúde, bem como, para o atendimento integrado às equipes. O estudo tem como objetivo descrever experiências de implementação e práticas das equipes de Cuidados Paliativos nas instituições hospitalares, descritas na literatura. Trata-se de revisão integrativa realizada em abril de 2021, através de pesquisa de artigos publicados na Biblioteca Virtual em Saúde, no período de janeiro de 2016 a março de 2021, utilizando-se como Descritores: cuidados paliativos, equipe multiprofissional, equipe de assistência ao paciente e hospital. Os estudos apontam para modelos de equipes de cuidados paliativos no formato de consultoria; de atendimento direto ao paciente por solicitação da equipe de saúde e; recentemente, em alguns países, integradas ao sistema e atuando autonomamente. Há resistência na implementação e atuação das equipes de cuidados paliativos em hospitais, especialmente por parte dos médicos, que também resistem em participar das equipes. Entretanto, os resultados alcançados têm contribuído para dirimir as barreiras e promover a evolução dessas equipes. Observa-se a tendência de abandonar os modelos exclusivamente consultivos e/ou a pedido, para torná-las equipes que atuam integradas em todos os níveis de atenção em saúde. Gradualmente tem sido reconhecida a importância da enfermagem para implementação e práticas das equipes de Cuidados Paliativos em hospitais.
