Creation of an institutional protocol for palliative care in a general hospital: a convergent care research

Resumo: In view of the need to diminish/amenify situations of physical and emotional suffering of patients and family members dealing with life threatening diseases, the objective of this study was to develop an institutional protocol of Palliative Care directed to the hospital environment. This is a research of qualitative approach of the assistential convergent type, from the experience of a Nursing Resident, together with the support of the Working Group on Palliative Care of a hospital in the interior of Rio Grande do Sul. Study groups and conversation rounds were held in order to define the team's objectives, criteria for the insertion of patients, their evaluations and follow-up, culminating in the creation of the protocol. It was defined that the group will act as a hospital inter-consultation team that will be activated upon the request of any senior health professional. The initial assessment of the patient will be done by a nurse, a physician and a psychologist, and the initial scales that will assess prognosis and symptoms respectively will be the Paliattive Performance Scale (PPS) and the Edmonton Symptom Assessment Scale (ESAS). A personalized discharge planning will also be carried out focusing on the patient's needs, actions to be directed to the family and activation of the care network. From this study one can expand research on the implementation of PC programs within the hospital environment, as well as modify the institutional culture, bringing to light such a relevant topic.

Se trata de una investigación de enfoque cualitativo del tipo convergente asistencial, a partir de la experiencia de una Residente de Enfermería, junto con el apoyo del Grupo de Trabajo en Cuidados Paliativos de un hospital del interior de Rio Grande do Sul. Se realizaron grupos de estudio y rondas de conversación para definir los objetivos del equipo, los criterios de inserción de los pacientes, sus valoraciones y su seguimiento, culminando con la creación del protocolo. Se definió que el grupo actuará como un equipo de interconsulta hospitalaria que se activará a petición de cualquier profesional sanitario superior. La primera evaluación del paciente será realizada por una enfermera, un médico y un psicólogo y las escalas iniciales que evaluarán el pronóstico y los síntomas, respectivamente, serán la Escala de Rendimiento Paliativo (PPS) y la Escala de Evaluación de Síntomas de Edmonton (ESAS). También se llevará a cabo una planificación personalizada del alta, centrada en las necesidades del paciente, las acciones que deben dirigirse a la familia y la activación de la red asistencial. A partir de este estudio, es posible ampliar la investigación sobre la implementación de programas de PC en el ámbito hospitalario, así como modificar la cultura institucional, sacando a la luz un tema tan relevante.Perante a necessidade de diminuir/amenizar as situações de sofrimento físico e emocional de pacientes e familiares que lidam com doenças ameaçadoras à vida, o objetivo deste estudo foi desenvolver um protocolo institucional de Cuidados Paliativos voltado ao ambiente hospitalar. Trata-se de uma pesquisa de abordagem qualitativa do tipo convergente assistencial, a partir da vivência de uma Residente de Enfermagem, juntamente com o apoio do Grupo de Trabalho em Cuidados Paliativos de um hospital do interior do Rio Grande do Sul. Grupos de estudo e rodas de conversa foram realizados a fim de definir os objetivos da equipe, critérios para inserção dos pacientes, suas avaliações e acompanhamento, culminando na criação do protocolo. Definiu-se que o grupo atuará como um time hospitalar de interconsulta que será acionado mediante solicitação de qualquer profissional da saúde de nível superior. A primeira avaliação do paciente será realizada por um profissional enfermeiro, um médico e um psicólogo e as escalas iniciais que avaliarão respectivamente prognóstico e sintomas serão a Paliattive Performance Scale (PPS) e a Edmonton Symptom Assessment Scale (ESAS). Também será realizado um planejamento de alta personalizado com foco nas necessidades do paciente, ações a serem orientadas à família e acionamento da rede de cuidados.
