Prevalencia das dermatoses e comorbidades em idosos atendidos em um ambulatorio de dermatologia

Resumo: | The number of elderly people in the brazilian population has been growing, reaching 14.3% of the population in 2015. The life expectancy in Rio Grande do Sul is 78.4 years in 2017. The factors responsible for this are the reduction of infant mortality, drop in fertility and increase in life expectancy. In the brazilian and world literature, there are few studies on the prevalence of skin diseases in the elderly. This study aims to characterize the epidemiological profile of skin changes and comorbidities that affect the elderly attended at the dermatology outpatient clinic of the Santa Maria’s University Hospital and the development of a check-list to help in the diagnosis of dermatosis in the geriatric dermatology care. A cross-sectional study was conducted analyzing data collected between May and August 2016, and included patients with ≥ 60 years old. A total of 349 patients were studied, predominantly female (54.2%), white (94.5%), incomplete elementary school (58.8%), farmer (51.6%) and 58.4% were phototype II. The most prevalent comorbidities were hypertension, dyslipidemia, diabetes mellitus and osteoporosis; in addition, we found that among of the 10 most prevalent dermatosis, 7 are associated with hypertension. Basal cell carcinoma was the most commonly neoplasm; the most prevalent skin diseases were actinic keratosis, cutaneous xerosis, seborrheic keratosis, wrinkles and poikiloderma. Based on the analyzed data, a check-list was set up to help in the diagnosis of the main skin diseases associated with each age group and the main comorbidities of this population. This study emphasizes the need for new research to be performed to better understand the dermatosis and comorbidities of the elderly. |

A expectativa de vida no Rio Grande do Sul é de 78,4 anos em 2017. Os fatores responsáveis por isso são a redução da mortalidade infantil, queda da fecundidade e aumento da expectativa de vida. Na literatura brasileira e mundial, existem poucos estudos sobre a prevalência das dermatoses nos idosos. Este estudo tem como objetivo caracterizar o perfil epidemiológico das alterações de pele e comorbidades que afetam os idosos atendidos no ambulatório de dermatologia do Hospital Universitário de Santa Maria e a elaboração de um check-list para auxílio no diagnóstico de dermatoses nas consultas de dermatologia geriátrica. Realizou-se um estudo transversal analisando dados coletados entre maio e agosto de 2016, sendo incluídos pacientes com idade ≥ 60 anos. Foram analisados 349 pacientes, predominando o sexo feminino (54,2%), raça branca (94,5%), ensino fundamental incompleto (58,8%), agricultor (51,6%) e 58,4% dos pacientes apresentavam fototipo II. As comorbidades mais prevalentes foram hipertensão arterial sistêmica (HAS), dislipidemia, diabetes mellitus e osteoporose, além disso, encontramos que dentre as 10 dermatoses mais prevalentes, 7 estão associadas com a HAS. O carcinoma basocelular foi a neoplasia mais encontrada, as dermatoses mais prevalentes foram ceratose actínica, xerose cutânea, ceratose seborreica, rugas e poiquilodermia. Com base nos dados analisados, montou-se um check-list para auxílio no diagnóstico das principais dermatoses associadas com cada faixa etária e as principais comorbidades dessa população.
