Distúrbios psíquicos menores em enfermeiros docentes

Resumo: This study investigates the psychological demand and job control (quadrants of the demandcontrol model) associated with minor psychiatric disorders (MPD) in nursing faculty. Sectional study including 130 nursing faculty members at the federal universities of Rio Grande do Sul, Brazil. For assessment of psychological distress and dimensions of the demand-control model, we used the Brazilian versions of the Self-Report-Questionnaire (SRQ-20) and the Job Stress Scale (JSS), respectively. Was used to enter data Epi Info ®, version 6.4, with independent double entry and analysis program SPSS ® Statistic (Predictive Analytics Software from SPSS Inc., Chicago - USA) version 18.0 for Windows. After the descriptive analysis, there were the bivariate and multivariable, adopting a level of 95% (p <0.05). The nursing professors were predominantly female (90.8%) with average age of 47 (± 4.65) years old, had a doctorate (56.9%), belonged to the white race (93.1%), were married or lived with a partner (74.6%), had one or two children (50%), had per capita family income of up to six times the minimum wage (50.5%) and had up to three dependents (73.8%). Regarding employment data, 50% of nursing professors belonged to the category professional assistant, followed by assistant category (36.9%). The system of work, 93.1% worked 40 hours per week full-time, 30% worked more than 20 years, developing teaching at undergraduate (50.8%) for undergraduate and postgraduate studies (49, 2%), research activities (91.5%) and extension (85.4%). The prevalence of minor psychiatric disorders was 20%. After adjustments for potential confounders, the odds of suspected mental disorder was higher in the quadrant active work (OR = 14.23, 95% CI 1.55 to 130.73), followed by highstrain quadrant (OR = 10.05; 95% CI = 1.23 to 82.44) compared to nursing professors in lowstrain quadrant. We conclude that high psychological demands and low job control may trigger bouts, including the MPD in nursing faculty. It is evident also that the high psychological demand has a negative influence on the work of nursing faculty members, even when these workers have high job control. Therefore, some individual strategies and organizational issues need to be rethought in order to prevent the mental illness of nursing professors.

Therefore, some individual strategies and organizational issues need to be rethought in order to prevent the mental illness of nursing professors.Esta pesquisa tem como objeto de estudo a associação entre altas demandas psicológicas e baixo controle do trabalhador sobre o trabalho e a ocorrência de Distúrbios Psíquicos Menores em enfermeiros docentes de universidades federais públicas. O estresse laboral pode resultar do desequilíbrio mantido entre as demandas psicológicas que o exercício profissional exige e a autonomia de decisão do trabalhador. Os Distúrbios Psíquicos Menores ocorrem quando existem alterações orgânicas significativas mediante a presença de um estressor. Este estudo objetiva investigar a demanda psicológica e o controle sobre o trabalho (quadrantes do Modelo Demanda-Controle) e sua associação com os Distúrbios Psíquicos Menores (DPM) em enfermeiros docentes. Estudo epidemiológico seccional incluindo 130 enfermeiros docentes das universidades federais do Rio Grande do Sul, Brasil. Para avaliação dos Distúrbios Psíquicos Menores e das dimensões do Modelo Demanda-Controle, foram utilizadas as versões brasileiras do Self-Report-Questionnaire (SRQ-20) e do Job Stress Scale (JSS), respectivamente. Foi utilizado para a inserção dos dados o programa Epi-info®, versão 6.4, com dupla digitação independente e para a análise o programa PASW Statistic® (Predictive Analytics Software, da SPSS Inc., Chicago - USA ) versão 18.0 for Windows. Após a análise descritiva, realizaram-se as análise bivariadas e multivariáveis, adotando-se níveis de confiança de 95% (p< 0,05) e de 75% (p ≤ 0,25), respectivamente. Os enfermeiros docentes eram predominantemente do sexo feminino (90,8%); com média da idade de 47 (±4,65) anos, tinham doutorado (56,9%), pertenciam à raça branca (93,1%), eram casados ou viviam com companheiro (74,6%); possuíam de um a dois filhos (50%); apresentavam renda familiar percapita de até seis salários mínimos (50,5%) e possuíam até três dependentes (73,8%). Quanto aos dados laborais, 50% dos enfermeiros docentes pertenciam à categoria profissional adjunto, seguidos pela categoria assistente (36,9%). Quanto ao regime de trabalho, 93,1% trabalhava 40 horas semanais com dedicação exclusiva, 30% trabalhava a mais de 20 anos; desenvolvendo atividades de ensino na graduação (50,8%), na graduação e na pós-graduação (49,2%) e atividades de pesquisa (91,5%) e extensão (85,4%). A prevalência de distúrbios psíquicos menores foi de 20%. Após ajustes por potenciais confundidores, a chance de distúrbios psíquicos foi maior no quadrante trabalho ativo (OR=14,23; IC95%= 1,55 130,73), seguido do quadrante alta exigência (OR=10,05; IC95%= 1,23 - 82,44), quando comparado aos enfermeiros docentes do quadrante baixa exigência. Conclui-se que a alta demanda psicológica e o baixo controle sobre o trabalho tem desencadeado acometimentos, dentre eles os DPM em enfermeiros docentes. Evidencia-se, ainda, que a alta demanda psicológica tem influência negativa no trabalho dos enfermeiros docentes, mesmo quando esses trabalhadores possuem alto controle sobre o trabalho.
