Resumo: The purpose of this article is to analyze the meanings of the nominal expression “Gender Incongruence”, a diagnostic category directed to Trans, Transsexual, Transgender and Transvestite people and which appears in the 11th edition of the International Statistical Classification of Diseases and Related Health Problems (ICD - 11). For this purpose, three passages were selected, in which the expression is found in this text. As a theoretical-methodological reference, the analysis will be anchored in the Semântica Histórica da Enunciação (SHE), a theory proposed by Guimarães (2002), a branch of semantics in which the opacity of language and its historical relationship with reality are assumed and which deals with meaning at the same time as linguistic, historical and relative to the subject who enunciates (GUIMARÃES, 2002). We work with concepts such as Temporalidade, Reescrituração, Articulação, Cena Enunciativa and Domínio Semântico de Determinação (DSD). The results found lead to meanings of pathologies, cisnormativity and gender stereotypes, although the purpose of ICD 11 was to be more inclusive and less pathologizing with the Trans community than its previous edition.
