Resumo: Objective: to analyze the spatial distribution of COVID-19 cases and deaths and their relationship with socioeconomic and health indicators in Maranhão. Methods: ecological study whose unit of analysis were 217 municipalities in Maranhão. Incidence, mortality, and lethality due to COVID-19 were estimated based on information from the State Health Department up to August 2022. Moran Global verified spatial autocorrelation with incidence, mortality, and lethality, Moran Local identified clusters of high and low risk. Results: COVID-19 took place in a heterogeneous and non-random way in the state of Maranhão, positive spatial autocorrelation of incidence, mortality, and lethality was verified. The variables that best explained the occurrence of COVID-19 were the Gini index, HDI, IVS, primary care coverage, proportion of poor people, household crowding and illiteracy. Conclusion: socioeconomic and health indicators influenced the incidence, mortality, and lethality by COVID-19 in the state and should be considered in public policy formation.
