Síndrome de Leigh: Relato de caso clínico

Resumo: | AIM: to describe the clinical case of a child diagnosed with Leigh Syndrome, its oral characteristics and to address the importance of dental monitoring, as well as behavioral management in face of the proposed treatment. CLINICAL CASE REPORT: The female patient, aged 7 years and 1 month, diagnosed with Leigh Syndrome, sought the Special Needs Patient Care Clinic for dental follow-up. The patient received the first dental care at 3 years and 6 months of age, being followed at the Dental Hospital of the Federal University of Uberlândia. At the general clinical examination it was observed that the child does not speak and does not walk, presents a tonicity of the muscles of the altered face to the clinical examination. The presence of a mixed dental dentition with the presence of a visible dental biofilm was also observed during the intraoral examination. In addition, it presented caries activity in deciduous and permanent teeth, as well as severe bruxism. The child presented a difficult behavior, requiring the use of active protective stabilization during the consultations. The proposed dental treatment was preventive sealants in healthy teeth, therapeutic sealant in decayed teeth at the level of enamel and restorative treatment in teeth with caries lesion in dentin. The control of the dental biofilm was enhanced in all the consultations, through oral hygiene orientation and supervised brushing with the mother. In relation to bruxism, there was an improvement in muscle hypotonia after initiation of medical treatment with botulinum toxin. CONCLUSION: The case of a child diagnosed with Leigh 's Syndrome was described, presenting the systemic characteristics, and with respect to the oral characteristics. The dental approach is complex in function of the clinical manifestations, the role of the dental surgeon is important in these cases to assist the maintenance of oral health. |

RELATO DE CASO CLÍNICO: A paciente do sexo feminino com a idade de 7 anos e 1 mês diagnostica- da com a Síndrome de Leigh, procurou a Clínica de Atendimento de Pacientes com Necessidades Especiais para acompanhamento odontológico. A paciente recebeu o primeiro atendimento odontológico aos 3 anos e 6 meses de idade, sendo acompanhada no Hospital Odontológico da Universidade Federal de Uberlândia. Ao exame clínico geral foi observado que a criança não fala e não anda, apresenta tonicidade dos músculos da face alterada ao exame clínico. Ao exame clínico intrabucal verificou-se presença de dentadura mista, com presença de biofilme dental visível, além disso, apresentou atividade de cárie em dentes decíduos e permanentes, bem como bruxismo severo. A criança apresentou um comportamento difícil, sendo necessário o uso de estabilização protetora ativa durante as consultas. O tratamento odontológico proposto foi selantes preventivo em dentes hígidos e tratamento restaurador em dentes com lesão de cárie em dentina. O controle do biofilme dentário era realizado em todas as consultas, por meio de orientação de higiene bucal e escovação supervisionada com a mãe. Em relação ao bruxismo houve uma melhora no quadro de hipotonia muscular após o início de tratamento médico com uso de toxina botulínica. CONCLUSÃO: O caso de uma criança diagnosticada com a Síndrome de Leigh foi descrito, apresentando as característica sistêmicas.
