Using a fiber glass post (FGP) and the diameter of the FGP is a difficult decision during the restorative procedure of the incisor endodontically treated without ferrule. Purpose. This study evaluated the accelerated fatigue resistance and the failure mode of severely structurally compromised central incisors without ferrule rehabilitated with resin-based composite associated or not with FGP with different diameters. Materials and Methods: Sixty bovine incisors with similar dimensions were divided into 6 groups (n = 10) generated by a combination of root canal preparation for FGPs with a specific drill: 1) the Whitepost System DC 0.5 (FGM); 2) the Whitepost System DC-E 0.5 (FGM); 3) the Whitepost System FIT 0.4 (FGM) and the use or not of the FGPs to restore the teeth. The bovine dental crown was cut, resulting in a 15 mm root. The root canal was endodontically treated and filled with endodontic cement (AH Plus; Dentsply Sirona). The root canal was relieved with 10 mm and prepared with specific drills for FGP, and the root dentin was treated using a universal self-etching adhesive system (Ambar APS Universal; FGM). Half of the teeth had different FGPs, cemented using dual-cure resin cement (Allcem Core; FGM,) and the other half had the root canal space filled with bulk-fill resin-based composite (OPUS Bulk Fill; FGM). The crown was restored with bulk-fill resin-based composite using a standardized transparent matrix. The teeth were embedded in polystyrene resin, and the periodontal ligament was simulated with polyether impression material (Impregum; 3M Oral Care). All the specimens were subjected to digital radiography, transillumination, and scanning with microcomputed tomography before and 6 after accelerated fatigue testing under submerged conditions at 25 oC. Cyclic isometric loading was applied to the incisal edge at an angle of 45 degrees with a frequency of 5 Hz, beginning with a load of 50 N (5000 cycles). A 100 N load increase was applied every 15000 cycles. The fracture mode was classified according to severity and repair potential. For pairwise comparisons, the data were analyzed with the Kaplan-Meier survival test and the post hoc log-rank test (α =.05). Results. Regardless of FGP diameters (P<.001), the use of FGP significantly increased the survival of restored teeth. Increasing the diameter of the FGP showed no statistical difference in fatigue survival ratio or failure mode. The groups restored without FGP frequently had bubbles at the apical limit of the root canal preparation. Conclusions. The use of FGP improved the fracture resistance of severely structurally compromised central incisors without ferrule rehabilitated with resin-based composite under accelerated fatigue. The diameter of the FGPs had no effect on the survival and failure modes.
