Histoplasmose oral disseminada em paciente imunodeprimido: relato de caso clínico

Resumo: | Histoplasmosis is a fungal infection caused by the microorganism Histoplasma capsulatum. Oral lesions usually occur in the disseminated form of the disease, in immunocompromised patients, with the tongue, the palate and oral mucosa being the commonly involved sites, presenting clinically as ulcers. The objective of the present study was to report a case of a patient affected by Histoplasmosis in the palate region, seeking to reinforce the importance of knowledge by the dentists about this disease, for the most correct and early diagnosis possible. Patient J. A. F. G., 40 years olds, attended the stomatology clinic of the Faculty of Dentistry of the Federal University of Uberlândia with complaint of dysphagia and odynophagia. An extensive predominantly reddish palatal lesion with white necrotic areas was identified. Clinical, laboratory and microscopic findings indicated the probable final diagnosis of Disseminated Oral Histoplamosis associated with the Acquired Immune Deficiency Virus positivity. The treatment chosen was intravenous Amphotericin B Deoxycholate and Itraconazole, with the final result being absolute palatal normality and in the other regions previously affected by the fungal lesion. The reported case highlights the importance of attention by the dentist to possible oral manifestations of systemic diseases, such as Histoplasmosis, so that the diagnosis can be established as early as possible and the patient receives appropriate treatment. |

As lesões orais costumam ocorrer na forma disseminada da doença, em pacientes imunodeprimidos, sendo a língua, palato e mucosa oral as regiões mais afetadas, apresentando-se clinicamente como úlceras. O objetivo do presente trabalho foi relatar um caso clínico de um paciente acometido por Histoplasmose em região de palato, buscando reforçar a importância do conhecimento por parte do cirurgião-dentista acerca dessa doença, para o diagnóstico mais correto e precoce possível. O paciente J. A. F. G., 40 anos de idade, compareceu ao ambulatório de estomatologia da Faculdade de Odontologia da Universidade Federal de Uberlândia com queixa de disfagia e odinofagia. Foi identificada uma extensa lesão palatal predominantemente avermelhada com áreas brancas necróticas. Os achados clínicos, laboratorias e microscópicos indicaram o provável diagnóstico final de Histoplamose Oral Disseminada, associada à positividade do Vírus da Imunodeficiência Adquirida. O tratamento escolhido foi Anfotericina B Desoxicolato intravenosa e Itraconazol, tendo como resultado final absoluta normalidade palatal e nas demais regiões antes acometidas pela lesão fúngica.
