Agulhamento a seco no tratamento de disfunção temporomandibular muscular: relato de caso clínico

Resumo: | Temporomandibular Disorder (TMD) is a set of alterations involving the temporomandibular joint (TMJ), mastication muscles and associated structures. This condition has a multifactorial etiology, such as anatomical, pathophysiological, psychosocial and traumatic factors, which can contribute to its development. TMD can be classified into muscle and temporomandibular joint dysfunction. One of the muscles TMD subtypes is myofascial pain, and through muscle palpation it is possible to identify nodules in a tense muscle band called trigger points, associated with spontaneous, spreading or referred orofacial pain. The treatment must present a conservative, multidisciplinary and multidisciplinary approach. Among the treatments for myofascial pain, dry needling can be performed. The objective of this study was to carry out a clinical case report using dry needling in the treatment of a patient with muscular TMD. For the scientific basis of this report, a search was carried out in databases such as PubMed/Medline, SciELO and Periodicals CAPES. Thus, present a clinical case report of a female patient, 47 years old, complaining of myalgia on the left side of her face, limited mouth opening and constant headache. The dry needling procedure was performed in the trigger point regions, associating pharmacotherapy, occlusal splint and thermotherapy. Subsequently, the patient reported an improvement in pain, confirmed in the clinical follow-up. The reviewed scientific literature reported clinical success in the use of dry needling in the treatment of muscle TMD, which may enhance the result by associating it with other therapeutic treatments. The treatment protocol used proved to be effective in controlling pain and TMJ range of motion. |

Essa condição apresenta etiologia multifatorial, tais como fatores anatômicos, fisiopatológicos, psicossociais e traumáticos, que podem contribuir para o seu desenvolvimento. A DTM pode ser classificada em desordem muscular e desordem da ATM. Um dos subtipos de DTM muscular é a dor miofascial, e por meio da palpação muscular é possível identificar nódulos numa faixa tensa do músculo denominados pontos-gatilho, associado à dor orofacial espontânea, com espalhamento ou referida. O tratamento deve apresentar uma abordagem conservadora, multiprofissional e multidisciplinar. Dentre os tratamentos da dor miofascial pode ser realizado o agulhamento a seco. O objetivo deste estudo foi realizar um relato de caso clínico utilizando o agulhamento a seco no tratamento de uma paciente com DTM muscular. Para o embasamento científico deste relato foi realizada uma busca em bases de dados como PubMed/Medline, SciELO e Periódicos CAPES. Dessa forma, apresenta-se o relato de caso clínico de uma paciente do sexo feminino, 47 anos de idade, com queixa de mialgia do lado esquerdo do rosto, com limitação de abertura bucal e cefaleia constante. Foi realizado o procedimento de agulhamento a seco nas regiões de pontos-gatilho associando a farmacoterapia, placa estabilizadora e termoterapia. Posteriormente a paciente relatou melhora do quadro álgico, confirmada no retorno clínico. A literatura científica revisada relatou sucesso clínico na utilização do agulhamento a seco no tratamento de DTM muscular, podendo potencializar o resultado associando a outros tratamentos terapêuticos.
