Fatores que interferem na produção e características do concentrado sanguíneo - PRF (Fibrina Rica em Plaquetas)

Resumo: The blood concentrates, represented mainly by platelet-rich plasma (PRP) and platelet-rich fibrin (PRF), have been used in several medical and dental areas due to the good clinical outcomes achieved, low cost, easy of preparation and handling. The good clinical outcomes observer occurred due to the ability of these concentrates to improve the repair of soft and hard tissues, due to the ation of the growth factors and cytokines inside these concentrates that are responsible for the body's homeostasis. The PRP is considered the first generation of blood concentrates and has been showed a limited effect on tissue repair. On the other hand, The PRF is considered a second generation of blood concentrates, has been showed advantages such as the absence of necessity of the use of pro- and anti-coagulation additives, and the obtation of this concentrate can be done with only one centrifugation cycle, which is not possible with the PRP. The three-dimensional fibrin mesh formed after obtaining the PRF concentrates presents a significant amount of platelets and leukocytes that in conjunction with the growth factors act to enhance the tissue repair. Currently, it is recognized that several factors can interfere with the quality of the PRF production. Thus, considering the importance of knowing the parameters related to the efficiency of PRF, the aim of this monograph was to address factors that interfere with the physical characteristics and biological properties of the PRF concentrate. In this literature review, articles and books were consulted in the Pubmed, Embase, Web of science and Scopus databases, from 2018 to 2023. The keywords used were: Platelets rich fibrin, PRF protocol, tubes, and relative centrifugal force. Specific papers obtained from the references cited in the articles that encompassed the period of evaluation proposed were also included. It was showed that several factors can interfere with the characteristics of the PRF. Among them, the Relative Force of Centrifugation (RCF), centrifugation time and material of the blood collection tube, in addition to the systemic characteristics of the patient deserves highlight . These factors are variables that directly interfere with the physical and biological properties of PRF, and it is important for professionals to know the characteristics of each PRF protocol and standardize its use according to clinical needs.

Os resultados clínicos devem-se à capacidade desses concentrados melhorarem o reparo de tecidos moles e duros, visto que fornecem fatores de crescimento e citocinas, responsáveis pela homeostase do organismo. O PRP, considerado a primeira geração de concentrados sanguíneos apresenta efeito limitado no reparo tecidual. Já o PRF, considerado a segunda geração de concentrados sanguíneos que, além de apresentar vantagens como ausência de aditivos pró e anti coagulantes e por ser obtido com apenas um ciclo de centrifugação, tem apresentado melhores resultados no reparo tecidual que o PRP. A malha tridimensional de fibrina formada após a obtenção do PRF concentra quantidade significativa de plaquetas e leucócitos que, em conjunto com os fatores de crescimento, atuam potencializando o processo de reparo tecidual. Atualmente, reconhece-se que vários fatores podem interferir na qualidade do PRF produzido. Assim, considerando a importância de conhecer os parâmetros relacionados à eficiência do PRF, o objetivo da presente monografia foi abordar fatores que interferem nas características físicas e propriedades biológicas do concentrado Fibrina Rica em Plaquetas. Na revisão de literatura, foram consultados artigos e livros, nas bases dados Pubmed, Embase, Web of science e Scopus, no período de 2018 a 2023. As palavras chaves utilizadas foram: Platelets rich fibrin, PRF protocol, tubes, relative centrifugal force. Também foram incluídos artigos específicos obtidos a partir das referências citadas nos artigos do período consultado. A literatura mostrou que vários fatores podem interferir nas características do PRF produzido. Entre eles, destacam-se a Força Relativa de Centrifugação (RCF), tempo de centrifugação e material do tubo de coleta de sangue, além das características sistêmicas do paciente.
