Total edentulous patients’ conventional prostheses users report clinical and daily problems, mainly with mandibular prostheses and present difficulties in speech and in their feeding, two essential activities to maintain the quality of life with negative impacts on the psychosocial well-being. Objectives: To evaluate the impact of total removable mandibular restorations with implants (overdentures) on the oral health-related quality of life of the senescent population. Methods: 11 patients aged 60–75 years old treated with overdenture (two implants and bar-clip system) at the Universidade Federal de Uberlândia in 2015–2018 were included. All patients answered the Oral Health Impact Profile (OHIP-14br) questionnaire before and after received the treatment. Also, was performed maximum force bite evaluation and registration of the bone leveling. Data regarding overdenture features and clinical factors were recorded. Descriptive probes, T paired tests, and the Spearman correlation coefficient were applied (p = 0.05). Results: All 11 patients answered the OHIP-14br questions and all items was properly filled in each case. The mean total OHIP-14br score was 6,9 ± 4,4 points before treatment and 2,99 ± 4 points after. The impact of oral heath in the quality of life was greater in domains as physical pain (impact = 1.3 ± 1.1), psychological discomfort (impact = 0.9 ± 1) and function limitation (impact = 1.3 ± 1.1). Maximum bite force was greater after the patient received treatment from 62,72 N to 106,03 N on the right side and from 61,12 N to 114,33 N on the left side (p < 0,05). Bone level around implant platform variation had no statically relevant difference (p > 0.05). Conclusions: Prostheses overdentures with bar-clip system provide a seemingly acceptable quality of life in the elderly population studied, increasing the maximum bite force and stability in mandibular function.
