High-Cost Drugs: Understanding the Management and Failures of Dispensation in Five Brazilian States

Resumo: Some medications are classified as high cost, with monthly expenses exceeding R$ 200,000.000. For the distribution of such drugs, the Ministry of Health publishes lists called exceptional dispensing drugs, federally funded with cash transfers to states and municipalities. However the access to medicines, under the Health System, presents various administrative obstacles. To understand the reasons for the failures of exceptional drugs distribution as well as the consequences for management in public health it was the objective of this survey. A documental analysis of official lists of five Brazilian states and other ordinances related to the dispensing of high-cost drugs was performed, in addition to content analysis of secondary sources as the lawsuits brought by patients with rare diseases, with the Public Prosecutor. The paper also discusses new proposals and guidelines for the management of the list of high-cost medicines state. Keywords: High-cost, drugs, failure, dispensation, health management.

Para a distribuição desses medicamentos, o Ministério da Saúde publica as chamadas listas de medicamentos de dispensação excepcional, financiadas pelo governo federal, com repasse aos estados e municípios. Porém, o acesso aos medicamentos no âmbito do Sistema Único de Saúde apresenta diversos obstáculos administrativos. Compreender os motivos das falhas de distribuição de medicamentos excepcionais, bem como as consequências para a gestão em saúde pública, foi o objetivo desse levantamento. Foi realizada uma análise documental das listas oficiais de cinco estados brasileiros e demais portarias referentes à dispensação de medicamentos de alto custo, além da análise de conteúdo de fontes secundárias, como as ações movidas pelos pacientes portadores de doenças raras junto ao Ministério Público. O trabalho ainda aborda novas propostas e diretrizes para a gestão da lista de medicamentos de alto custo estaduais. Palavras-chave: medicamentos, alto custo, falhas, dispensação, gestão em saúde.
