Al?m disso, a mastiga??o exerce influ?ncia na digest?o e absor??o de nutrientes, fundamentais para o crescimento e manuten??o da sa?de. Tal investiga??o ainda ? limitada atrav?s de estudos longitudinais. Essa tese foi desenvolvida na forma de dois artigos cient?ficos. O primeiro artigo teve como objetivo determinar os fatores de risco para a piora da Performance Mastigat?ria (PM) de pr?-escolares. Um estudo de coorte foi realizado com 136 pr?-escolares selecionados a partir de um estudo transversal (baseline). As crian?as e suas m?es/respons?veis foram contatadas ap?s um ano do baseline para uma segunda avalia??o. Todas as vari?veis foram coletadas em ambas as avalia??es. O exame cl?nico bucal foi realizado para avalia??o de m?-oclus?o posterior, do n?mero de dentes perdidos devido ? c?rie e da presen?a de c?rie dent?ria. A presen?a de c?rie foi avaliada atrav?s do Sistema Internacional de Avalia??o e Detec??o de C?rie Dent?ria (ICDAS II). As m?es preencheram um recordat?rio alimentar de 3 dias das crian?as, que foi usado para c?lculo da frequ?ncia m?dia di?ria de ingest?o de alimentos s?lidos, l?quidos e pastosos. Para avalia??o da PM, foi utilizado um alimento teste artificial (Optocal). O m?todo de processamento deste alimento se deu por peneiramento, atrav?s de nove peneiras acopladas a um agitador mec?nico por 20 minutos. As part?culas retidas em cada peneira foram pesadas e a partir desse peso foi determinado o tamanho mediano (X50) das part?culas trituradas para cada crian?a. Todas as vari?veis foram categorizadas de acordo com sua varia??o durante o per?odo de acompanhamento. O risco de impacto das vari?veis sobre a piora da PM foi testado por meio da regress?o hier?rquica de Poisson. O aumento do n?mero de dentes cavitados (RR= 1,98; IC95%= 1,01-3,93), aumento do n?mero de dentes posteriores perdidos (RR= 3,29; IC95%= 1,67-6,47) e redu??o da frequ?ncia m?dia di?ria de consumo de alimentos s?lidos (RR= 2,29; IC 95%= 1,22-4,31) foram associados a uma piora da PM. O objetivo do segundo estudo foi avaliar o efeito do tratamento restaurador na performance mastigat?ria (PM) de pr?-escolares com les?es cariosas cavitadas posteriores, al?m de comparar a PM ap?s o tratamento com um grupo com denti??o saud?vel, que n?o recebeu interven??o. Este estudo longitudinal com interven??o em um grupo foi realizado com 58 crian?as de 4 e 5 anos que apresentavam les?es cariosas cavitadas nos dentes posteriores, compondo o grupo interven??o. Essas crian?as foram pareadas com crian?as da mesma idade e sexo que apresentavam os dentes posteriores h?gidos, para forma??o do grupo n?o interven??o. A avalia??o foi realizada em dois momentos: previamente ao in?cio do tratamento restaurador das crian?as do grupo interven??o e 15 dias ap?s finalizado o tratamento. A avalia??o da PM foi realizada utilizando o alimento teste artificial optocal. O m?todo utilizado para c?lculo do tamanho mediano das part?culas (X50) foi o peneiramento. Houve diferen?a do valor do X50 entre os dois grupos tanto na primeira avalia??o (p=0,003), quanto no acompanhamento (p=0,020), onde o grupo n?o interven??o apresentou melhor PM. No entanto, o delta do X50 foi menor no grupo interven??o (p<0,001) (maior melhora). No modelo final da regress?o m?ltipla, o ?X50 foi influenciado pelo n?mero de dentes restaurados (?=-0,391; p<0,001). O tratamento restaurador apresentou efeito positivo para a melhora da PM. Houve uma maior varia??o para melhor da PM do grupo interven??o. O aumento do n?mero de dentes perdidos e cavitados s?o fatores de risco para a piora da PM de pr?-escolares Al?m disso, ? importante avaliar a consist?ncia dos alimentos oferecidos, uma vez, que quando h? uma redu??o da frequ?ncia de ingest?o de alimentos s?lidos, ocorre uma tend?ncia ? piora da PM. Ap?s o tratamento restaurador odontol?gico houve uma melhora significativa da PM. Entretanto, n?o foram alcan?ados n?veis semelhantes ?s crian?as com todos os dentes posteriores h?gidos. In addition, chewing exerts influence on the digestion and absorption of nutrients, fundamental for growth and maintenance of general health. Such investigation is still limited by longitudinal studies. This thesis was developed in the form of two scientific papers. The first article had as objective to determine the risk factors for the worsening of the Masticatory Performance (MP) of preschool children. A cohort study was conducted with 136 preschoolers selected from a cross-sectional study (baseline). The children and their mothers / guardians were contacted after a baseline year for a second evaluation. All variables were collected in both evaluations. The oral clinical examination was performed to evaluate posterior malocclusion, the number of teeth lost due to caries and the presence of dental caries. The presence of caries was evaluated through the International System of Evaluation and Detection of Dental Caries (ICDAS II). Mothers completed a 3-day infant food recall, which was used to calculate the mean daily intake frequency of solid, liquid and pasty foods. For evaluation of MP, an artificial test food (Optocal) was used. The method of processing this food was by sieving through nine sieves coupled to a mechanical stirrer for 20 minutes. The particles retained in each sieve were weighed and from that weight the median size (X50) of the crushed particles for each child was determined. All variables were categorized according to their variation during the follow-up period. The risk of impact of the variables on the worsening of the MP was tested through the hierarchical Poisson regression. The increase in the number of caved teeth (RR = 1.98, 95% CI = 1.01-3.93), increase in the number of posterior teeth lost (RR = 3.29, 95% CI = 1.67-6.47 ) and reduction of the average daily frequency of solid food intake (RR = 2.29, 95% CI = 1.22-4.31) were associated with a worsening of MP. The objective of the second study was to evaluate the effect of restorative treatment on the masticatory performance of pre-schoolers with posterior cavitated carious lesions and to compare MP after treatment with a group with healthy dentition that did not receive intervention. This longitudinal study with intervention in a group was performed with 58 children of 4 and 5 years who presented carious lesions cavitated in the posterior teeth, composing the intervention group. These children were paired with children of the same age and sex who had healthy posterior teeth, to form the nonintervention group. The evaluation was performed in two moments: before the beginning of the restorative treatment of the children of the intervention group and 15 days after the end of the treatment. MP evaluation was performed using the artificial optocal test food. The method used to calculate the median particle size (X50) was sieving. There was a difference in the value of X50 between the two groups, both in the first evaluation (p = 0.003) and in the follow-up (p = 0.020), where the nonintervention group had a better MP. However, the X50 delta was lower in the intervention group (p <0.001) (greater improvement). In the final multiple regression model, ?X50 was influenced by the number of restored teeth (? = -0.391, p <0.001). The restorative treatment had a positive effect on MP improvement. There was a greater variation for better MP of the intervention group. The increase in the number of lost and cavitated teeth are risk factors for the worsening of the MP of preschoolers. In addition, it is important to evaluate the consistency of the food offered, once, that when there is a reduction in the frequency of intake of solid foods, there is a tendency for MP to worsen. After dental restorative treatment there was a significant improvement in MP. However, similar levels were not achieved for children with all posterior healthy teeth.
