Razão triglicerídeos/HDL em idosos atendidos na Estratégia Saúde da Família de Viçosa-MG

Resumo: Cardiovascular disease is a condition that is affecting many elderly, accounting for an increase in the prevalence of mortality in this age group. Knowing the factors involved in the pathology becomes relevant in order to prevent and treat it. The research objective was to identify cutoff points of anthropometric measures to predict cardiovascular risk in elderly, besides detecting factors associated with this risk. It is a cross-sectional study with elderly pattients in the Family Health Strategy of Viçosa, Minas Gerais. The cardiovascular risk was calculated by the ratio between the levels of triglycerides and HDL-cholesterol, and it is considered a value in cardiovascular risk greater than 3.5. In the first meeting, a questionnaire was given to socio-demographic information, lifestyle assessment, blood pressure measurement, test for the presence of depressive symptoms, and anthropometric measurements as body mass index, waist circumference, waist hip, waist height and conicity index. In a second meeting, biochemical tests were performed. To identify cutoffs anthropometric measurements to predict cardiovascular risk, it was used the student t test to compare means of independent groups with normal distribution and the Mann-Whitney test for nonparametric variables with distribution. The anthropometric measurements were compared with cardiovascular risk also through the Receiver Operating Characteristic curves. To assess risk factors for cardiovascular risk, it was performed by multiple linear regression analysis. For males, all indexes can be used to predict cardiovascular risk. However, in women, only body mass index was able to predict. The independent variables were gender, age, marital status, work status, education, ethnia, smoking, exposure to drinking, physical activity level, family history of chronic diseases, depressive symptoms, sedentary behavior, glucose levels and blood pressure. It was used a multiple regression, with p value <0.05. The elderly of higher social class, ex- smokers, with high glucose and history of heart disease were the most likely to have cardiovascular risk. These findings reflect the importance of studies on cardiovascular risk in the elderly, should the health professionals be familiar with the parameters that classify individuals with cardiovascular risk factors, and the relationship between triglycerides and HDL-cholesterol is an easy method, noninvasive and reliable to intervence in a better quality of life for seniors.

Conhecer os fatores envolvidos com a patologia torna-se relevante, a fim de preveni-la e tratá-la. Diante disso, objetivou-se identificar pontos de corte de medidas antropométricas com o intuito de avaliar a capacidade preditiva dessas medidas na identificação do risco cardiovascular elevado em idosos, além de detectar fatores associados ao referido risco. Foi conduzido um estudo do tipo transversal com idosos atendidos na Estratégia Saúde da Família do município de Viçosa, Minas Gerais. O risco cardiovascular elevado foi calculado pela relação entre os níveis de triglicerídeos sobre os níveis de HDL-colesterol, sendo considerado em risco cardiovascular elevado idosos com a razão TG/HDL maior que 3,5. A coleta de dados envolveu a aplicação de um questionário com informações sociodemográficas, avaliação dos hábitos de vida, aferição da pressão arterial, teste para verificação da presença de sintomas depressivos, além de índices antropométricos como índice de massa corporal, perímetro da cintura, relação cintura quadril, relação cintura estatura e índice de conicidade. Além disso, foram realizados exames bioquímicos. Para identificar pontos de corte das medidas antropométricas para predizer risco cardiovascular elevado, foi utilizado o teste t de Student para comparar as médias de grupos independentes com distribuição normal e o teste de Mann-Whitney para as variáveis com distribuição não paramétrica. A capacidade preditiva das medidas antropométricas para o risco cardiovascular elevado também através das curvas Receiver Operating Characteristic. Para associar fatores de risco para risco cardiovascular elevado, foi realizada pela análise de regressão linear múltipla. Para o sexo masculino, todos os índices antropométricos podem ser utilizados para predizer risco cardiovascular elevado. Porém, no sexo feminino, apenas o índice de massa corporal apresentou apto para a predição. Maiores valores da relação triglicerídeos sobre os níveis de HDL- colesterol associaram-se com a presença da hipertensão arterial, história prévia ou atual de exposição ao fumo, presença de níveis séricos elevados de glicose e possuir maiores perímetros da cintura.
