Financial performance in the supplementary health sector: an analysis of the variance of the profitability of health plan operators in Brazil

Resumo: The supplementary health market in Brazil is regulated by the National Supplementary Health Agency. The Agency is responsible, among others, for promoting the defense of the public interest in supplementary health care and contributing to the development of health actions in the country. To fulfill its duties, the regulator must keep the information base continuously updated. Although the Agency does not directly regulate prices in the market, it must monitor the financial performance of health plan operators - OPS. This study provides an analysis of the return of 14 OPS in Brazil, in the period 2007-2016, using the technique of decomposition of the return on asset variance. The modeling is based on the pioneering study by Schmalensee(1). Four effects were tested: scope, flag, corporate and year. The most important innovation of the study is to introduce the scope effect as a proxy for economies of scale and/or scope in the activity. It was found that the scope effect is considerably more important for the profitability of operators, followed by the flag effect; the year effect was significant, but relatively small; finally, the corporate effect was statistically insignificant.

Entre sus funciones la Agencia es la responsable de promover la defensa del interés público en la atención complementaria de salud y contribuir al desarrollo de las acciones de salud en el país. Para cumplir con sus funciones, el regulador debe mantener la base de información continuamente actualizada. Aunque la Agencia no regula directamente los precios en el mercado debe monitorear el desempeño financiero de los operadores de planes de salud - OPS. Este estudio proporciona un análisis de la rentabilidad de 14 OPS en Brasil, en el período 2007-2016, utilizando la técnica de descomposición del retorno sobre la varianza de los activos. El modelado se basa en el estudio pionero de Schmalensee(1). Se probaron cuatro efectos: alcance, bandera, corporativo y año. La innovación más importante del estudio es introducir el efecto de alcance como un sustituto de las ganancias en escala y/o alcance en la actividad. Se encontró que el efecto de alcance es considerablemente más importante para la rentabilidad de los operadores, seguido del efecto bandera; el efecto del año fue significativo, pero relativamente menor; finalmente, el efecto corporativo fue estadísticamente insignificante.O mercado de saúde suplementar no Brasil é regulado pela Agência Nacional de Saúde Suplementar. Este estudo traz uma análise da rentabilidade de 14 OPS no Brasil, no período de 2007-2016, por meio da técnica de decomposição da variância do retorno sobre ativo. A modelagem baseia-se no estudo pioneiro de Schmalensee(1). Foram testados quatro efeitos: abrangência, bandeira, corporativo e ano. A inovação mais importante do estudo consiste em introduzir o efeito abrangência como proxy para ganhos de escala e/ou de escopo na atividade.
