The relational dimension of governance in the regionalization of health: : perspectives of regional managers of the Macrorregião de Saúde Sul

Resumo: The objective of this study was understand, from the point of view of managers at the regional level of the Southern Health Macroregion of Rio Grande do Sul, which are the potentializing and limiting elements that impact on regional governance, in the relational dimension. Through a qualitative approach, semi-structured interviews were executed with two regional health managers intentionally chosen for their trajectory in the territory. As a result, the importance of regional health managers who know the Sistema Único de Saúde and regionalization, who understand themselves as part of the territory and value this instance was highlighted. The high degree of communication and collaboration between regional and municipal health managers appeared as a facilitator of regional governance, as well as itinerant meetings and informal moments. However the high turnover of managers emerged as limiter on regionalization processes, once the composition of decision groups influences the relational dimension and reflects on inter-federal pacts. The micropolitical phenomena found build the scenario of agreement and impact on the direction of public policies in the territory. Thus, understanding these relational factors can be a powerful tool to guide health management and planning strategies.

Thus, understanding these relational factors can be a powerful tool to guide health management and planning strategies.El objetivo de este estudio fue averiguar, desde el punto de vista de los gestores a nivel regional de la Macrorregión de Salud Sur de Rio Grande do Sul, cuáles son los elementos potencializadores y limitantes que impactan en la gobernanza regional, en la dimensión relacional. A través de un enfoque cualitativo, se realizaron entrevistas semiestructuradas a dos gestores regionales de salud elegidos intencionalmente por su trayectoria en el territorio. Como resultado, se destacó la importancia de gestores regionales de salud que conozcan el Sistema Único de Salud y la regionalización, que se comprendan como parte del territorio y valoren esta instancia. El alto grado de comunicación y colaboración entre los gestores regionales y municipales de salud apareció como un facilitador de la gobernanza regional, así como de encuentros itinerantes y momentos informales. Por otro lado, la alta rotación de gerentes surgió como una limitante en los procesos de regionalización, ya que la composición de los grupos de decisión influye en la dimensión relacional y refleja en los pactos interfederales. Los fenómenos micropolíticos encontrados hacien a construcción del arena de acuerdo e influencian en la dirección de las políticas públicas en el territorio. Así, comprender estos factores relacionales puede ser una herramienta poderosa para guiar las estrategias de gestión y planificación de la salud.O objetivo do presente estudo foi conhecer, a partir do olhar dos gestores de nível regional da Macrorregião de Saúde Sul do Rio Grande do Sul, quais são os elementos potencializadores e limitadores que impactam na governança regional, na dimensão relacional. Através de abordagem qualitativa foram realizadas entrevistas semiestruturadas com dois gestores regionais de saúde escolhidos intencionalmente pela sua trajetória no território. Como resultado, destacou-se a importância de gestores regionais de saúde que conheçam o Sistema Único de Saúde e a regionalização, que se entendam como parte do território e valorizem essa instância. O alto grau de comunicação e colaboração entre gestores regionais e municipais de saúde apareceu como um elemento facilitador da governança regional, assim como as reuniões itinerantes e os momentos informais. Por outro lado, a alta rotatividade de gestores emergiu como um limitador dos processos da regionalização, uma vez que a composição dos grupos de decisão influencia a dimensão relacional e reflete nas pactuações interfederativas. Os fenômenos micropolíticos encontrados constroem o cenário de pactuação e impactam no direcionamento das políticas públicas no território.
