Study of the complementary tests requested by the Primary Health Care in a large municipality

Resumo: The unqualified request for complementary examinations in medical consultations has become incompatible with the availability of the Unified Health System (SUS), generating long-term scheduling, requiring intervention strategies. In this direction, the "Corujão da Saúde" Program, in 2017, of the São Paulo Municipal Health Secretariat (SMS-SP), aims to clear waiting queues for certain exams requested in the basic care network. This work characterized the pertinence of these requests included in this Program. It was a cross-sectional and quantitative study, from a sample of 2790 exams, from the Basic Health Units, Regional Health Coordination Center / SMS-SP. The variables selected were: type of exam, date of birth of the user, date of order, diagnostic hypothesis. We used Excel-Microsoft 2010 for data storage and reporting. Of the exams, 73% had a waiting period of 6 to 12 months. Most are users between the ages of 18 and 50. Ultrasonography represented 88% of the total, and bilateral breast ultrasonography predominated. It is evidenced the inadequate use of the examination in the medical consultations of the primary care, which reinforces the importance of, implantation of clinical protocols, contributing in the prevention of iatrogenias and an efficient rationality of the health system.

En esta dirección, surge el Programa "Corujão da Saúde", en 2017, de la Secretaría Municipal de Salud de São Paulo (SMS-SP), con el objetivo de poner a cero las colas de espera de determinados exámenes solicitados en la red de atención básica. Este trabajo caracterizó la pertinencia de estas solicitudes incluidas en este Programa. Fue un estudio transversal y cuantitativo, a partir de una muestra de 2790 exámenes, procedentes de las Unidades Básicas de Salud, Coordinadora Regional de Salud Centro / SMS-SP. Las variables seleccionadas fueron: tipo de examen, fecha de nacimiento del usuario, fecha de solicitud, hipótesis diagnóstica. Se utilizó Excel-Microsoft 2010 para almacenamiento de datos y confección de informes. De los exámenes, el 73% presentó una espera de 6 a 12 meses para la realización. La mayoría son usuarios entre de 18 a 50 años. Las ultrasonografías representaron el 88% del total, siendo que la ultrasonografía mamaria bilateral las predominante. Se evidencia la utilización inadecuada de examen en las consultas médicas de la atención primaria, lo que refuerza la importancia de, implantación de protocolos clínicos, contribuyendo en la prevención de iatrogenias y una racionalidad eficiente del sistema de salud.A solicitação não criteriosa de exames complementares nas consultas médicas tem se tornado incompatível com a disponibilidade do Sistema Único de Saúde (SUS), gerando agendamento a longo prazo, exigindo estratégias de intervenção. Nessa direção, surge o Programa “Corujão da Saúde”, em 2017, da Secretaria Municipal da Saúde de São Paulo (SMS-SP), visando zerar as filas de espera de determinados exames solicitados na rede de atenção básica. Este trabalho caracterizou a pertinência dessas solicitações incluídas neste Programa. Foi um estudo transversal e quantitativo, a partir de uma amostra de 2790 exames, procedentes das Unidades Básicas de Saúde, Coordenadoria Regional de Saúde Centro/SMS-SP.
