O tratamento da SAHOS tem por objetivo normalizar a respiração durante o sono e proporcionar uma boa qualidade de vida ao paciente. A cirurgia ortognática é uma ótima opção de tratamento para os pacientes com SAHOS que não se adaptaram ao CPAP e apresentam deficiência ântero-posterior maxilar e mandibular. O objetivo do artigo é relatar o caso de um paciente com SAHOS moderada tratado com avanço maxilomandibular. Relato do caso: Paciente A.O.F., gênero masculino, 38 anos de idade, diagnosticado com síndrome da apneia e hipopneia obstrutiva do sono apresentando índice de apneia/hipopneia de 19,69/hora. Foi realizado o exame físico do paciente, constatando deficiência ântero-posterior de maxila e mandíbula. O exame de polissonografia confirmou a presença de eventos obstrutivos noturnos e a tomografia mostra um estreitamento das vias aéreas. Foi realizada a cirurgia de avanço bimaxilar e as condições pré-existentes foram corrigidas. The treatment of OSAHS aims to normalize breathing during sleep and provide a good quality of life for the patient. Orthodontic surgery is a great treatment option for patient with OSAHS who do not adapt to CPAP and presents an anteroposterior maxillary and mandibular deficiency. The article aims to report the case of a patient with moderate OSAHS treated with maxillomandibular advancement. Case report: A.O.F. patient, male, 38 years old, diagnosed with obstructive sleep apnea and hypopnea syndrome presenting an apnea/hypopnea index of 19.69/hour. The patient's physical exam was performed, finding anteroposterior maxillomandibular deficiency. The polysomnography examination confirmed the presence of nocturnal obstructive events and the CT scan shows a narrowing of the airways. Bimaxillary advancement surgery was performed and pre-existing conditions were corrected.
