An approach to fatphobia present in health services

Resumo: Objective: To analyze the content of reports posted on social media on the topic of medical fatphobia. Methods: This study examined publicly available retrospective data on the social networks Twitter and Instagram, between January 2021 and January 2022. Each selected publication was categorized according to gender, grammatical person of the reports, main theme and consequences the health. Results: 75 reports were analyzed, mostly female (86.7%), mentioning 13 health specialties. Unsolicited encouragement of surgical measures for weight loss, lack of attention to patients' main complaints were reported; psychological, verbal and/or physical violence associated with being overweight and, in general, fatphobia as a barrier to accessing health care. Regarding the categorization of health consequences, reports of treatment abandonment, damage to mental health and consequences for maternal and child health were found. Conclusion: The characteristics of the reports are consistent with the discussions described in other international studies, such as the underrepresentation of men in debates related to obesity, the hegemony of biomedical knowledge in the face of social issues, the narrowing of access to health and the negative contribution to people's physical and mental health.

Métodos: este estudio examinó datos retrospectivos disponibles en las redes sociales Twitter e Instagram, entre enero de 2021 y enero de 2022. Cada publicación seleccionada fue categorizada según género, persona gramatical de los informes, tema principal y consecuencias para la salud. Resultados: Se analizaron 75 informes, en su mayoría femeninos (86,7%), mencionando 13 especialidades de la salud. Se reportaron estímulos no solicitados de medidas quirúrgicas para la pérdida de peso, falta de atención a las principales quejas de los pacientes; la violencia psicológica, verbal y/o física asociada al sobrepeso y, en general, la gordofobia como barrera para acceder a la atención sanitaria. Se encontraron relatos de abandono del tratamiento, daños a la salud mental y secuelas en la salud materno-infantil. Conclusión: Las características de los informes son consistentes con las discusiones descritas en otros estudios internacionales, como la subrepresentación de los hombres en los debates relacionados con la obesidad, la hegemonía del conocimiento biomédico frente a los problemas sociales, el estrechamiento del acceso a la salud y la contribución negativa a la salud física y mental de las personas.Objetivo: categorizar as vivências de gordofobia médica em serviços de saúde no Brasil. Método: estudo retrospectivo e misto, com abordagem exploratório sequencial, realizado entre janeiro de 2021 e janeiro de 2022. Os dados foram coletados manualmente, por meio da ferramenta de busca avançada e através da hashtag “gordofobia médica”, no Twitter e no Instagram, respectivamente. E foram analisados por meio de análise temática. Resultados: encontrou-se 476 registros e 75 foram selecionados. Teve-se como categorias temáticas, estímulo não solicitado a medidas cirúrgicas para o emagrecimento; falta de atenção às queixas principais dos pacientes; violências psicológicas, verbais e/ou físicas associadas ao excesso de peso; e gordofobia como barreira para o acesso à saúde. Além disso, foram relatadas percepções de consequências à saúde como, abandono de tratamento, prejuízos à saúde mental e prejuízos à saúde materno-infantil.
