O NR3C1, gene que codifica para a proteína receptora de glicocorticoides, e que é, portanto, implicado na resistência aos glicocorticoides, apresenta polimorfismos de nucleotídeo único (SNPs) que têm sido associados a doenças associadas ao estresse. A fim de investigar a relação entre os SNPs de NR3C1 e a suscetibilidade à DG, utilizamos uma combinação de levantamento da literatura e simulações de bioinformática para selecionar os SNPs rs104893913, rs104893909, rs104893911 e rs6198 que foram analisados em um total de 792 indivíduos, incluindo 384 pacientes com DG (325 mulheres e 59 homens, média de 41±13 anos), sendo 209 pacientes com Oftalmopatia de Graves (OG) e 408 controles saudáveis pareados (337 mulheres e 71 homens, média de 41±12 anos). Eventos estressantes foram avaliados através do questionário de autorrelato IES-R em 59 pacientes com DG e 66 controles saudáveis. Todos os SNPs apresentaram baixa frequência e perfil semelhante entre pacientes com DG e controles. No entanto a presença de rs6198 foi mais rara em pacientes com DG (21,6%) do que em controles (31,4%; p = 0,0019) representando um fator de proteção para DG (OR=0,6032; IC 95%: 0,4406-0,8275), mas sem influência no risco de OG ou qualquer outra característica clínica da doença. Eventos estressantes foram identificados em 69,5% dos pacientes e 37,9% dos controles (p=0,0004 OR=3,736; IC 95%: 1,753-8,049) e foram relatados imediatamente antes do início dos sintomas de DG em 23 pacientes, porém não conseguimos estabelecer qualquer associação entre esses eventos e genótipos de rs6198 ou características/fatores de risco de DG/OG. Sugerimos que o polimorfismo rs6198 do gene NR3C1 pode ser um fator protetor importante contra a suscetibilidade à DGAbstract: Although stressful events are admittedly related to Graves’ disease (GD) onset, the mechanisms that underlie this effect are still obscure. NR3C1 gene encodes glucocorticoid receptor protein, and therefore is implicated in glucocorticoid resistance, presents single nucleotide polymorphisms (SNPs) that have been reported in diseases associated with stress. Aiming to investigate the relationship between NR3C1 SNPs and susceptibility to GD, we used a combination of a literature survey and bioinformatics simulations to select rs104893913, rs104893909, rs104893911 and rs6198, which were analyzed in a total of 792 individuals, including 384 GD patients (325 women and 59 men, 41±13 years old), among which 209 patients presented Graves’ orbitopathy (GO), and 408 paired healthy controls (337 women and 71 men, 41±12 years old). Stressful life events were evaluated by the self-report questionnaire IES-R in 59 patients with GD and 66 healthy controls. All SNPs had a low frequency and a similar profile in GD patients and controls. However, variant forms of rs6198 were rarer in GD patients (21.6%) than in controls (31.4%; p=0.0019), representing a protective factor for GD (OR=0.6032; 95% CI: 0.4406-0.8275) but with no influence on GO risk or any association with other clinical features of the disease. Stressful events were identified in 69.5% of the patients and 37.9% control subjects (p=0.0004; OR=3.736; CI 95%: 1.753-8.049) and were reported to have occurred immediately before the onset of GD symptoms in 23 patients. However, we were unable to establish any association between these events and rs6198 genotypes or GD/GO characteristics or risk factors.
