Atresia bilateral de coanas em gêmeos prematuros monozigóticos e seu respectivo tratamento

Resumo: OBJECTIVES: To report a clinical case of bilateral choanal atresia in premature monozygotic twins and demonstrate the surgical treatment performed. METHODOLOGY: This is a descriptive and retrospective study of premature monozygotic twins patients with a clinical condition of bilateral choanal atresia. DESCRIPTION: Female univithelinous twins, born at 36 weeks and 5 days of gestation, to non-consanguineous parents, diagnosed with bilateral choanal atresia. The suspicion was presented by the inability to pass the nasal aspiration tube shortly after birth. They were referred to the Neonatal Intensive Care Unit (NICU) under ventilatory support. Paranasal 6 sinus tomography was requested, indicating complete bilateral choanal atresia in both patients. Surgical treatment was indicated through the transnasal endoscopic approach without the use of postoperative stents. Surgical correction was carried out without complications and without the need for redilatation. Currently, patients are 12 months old, with no late complications from the surgical approach. CONCLUSION: Complete bilateral choanal atresia is a condition of severe nasal obstruction, in which it can be diagnosed shortly after birth through cyclic cyanosis and the nasal tube cannot be introduced. Since it is a potentially fatal condition, surgical treatment must be quick and effective in order to make the airways viable. Bilateral choanal atresia is an extremely rare pathology in monozygotic twins, this being the third case reported in the literature.

METODOLOGIA: Trata-se de um estudo descritivo e restrospectivo de pacientes gêmeos prematuros monozigóticos com quadro clínico de atresia bilateral de coanas. DESCRIÇÃO: Gêmeos univitelinos do sexo feminino, nascidos com 36 semanas e 5 dias de gestação, de pais não consanguíneos, diagnosticados com atresia bilateral de coanas. A suspeita se apresentou pela incapacidade de passagem da sonda nasal de aspiração logo após o nascimento. Foram encaminhados à Unidade de Terapia Intensiva Neonatal (UTIN) sob suporte ventilatório. Foi solicitada a tomografia de seios paranasais, indicando a atresia bilateral completa de coanas nos dois pacientes. O tratamento cirúrgico foi indicado através da abordagem endoscópica transnasal sem uso de stents pós-operatórios. A correção cirúrgica foi realizada sem intercorrências e sem necessidade de redilatação. Atualmente, os pacientes estão com 12 meses de vida, sem complicações tardias da abordagem cirúrgica. CONCLUSÃO: A atresia bilateral completa de coanas é um quadro de obstrução nasal grave, que pode ser diagnosticado logo após ao nascimento através de cianose cíclica e inviabilidade de introdução da sonda nasal. Visto ser uma condição potencialmente fatal, o tratamento cirúrgico deve ser rápido e efetivo, a fim de viabilizar as vias aéreas.
