The objective of the study was to evaluate the Reception with Risk Classification for the elderly in a 24-hour Emergency Care Unit, under the perception of the elderly, family members and health professionals. To this end, a qualitative study was carried out, based on the theoretical and methodological framework of the Fourth Generation Assessment, with a 24-hour Emergency Care Unit in a municipality in the northwest of Paraná-Brazil as its research location. Data were collected during the months from February to September 2017, through information and negotiation meetings. The study included 33 social actors, divided into 13 health professionals, 12 elderly people and eight elderly family members. There were limitations in the application of the Manchester protocol in the Reception with Risk classification due to the bias of understanding senility and senescence in the emergency service, in addition to internal challenges due to the dynamics of the service, such as lack of training to assist this population in addition to the inconsistency of the information system itself. Through the analysis of the data obtained, it became evident that the approach to care for the elderly in the urgency and emergency service is incipient, requiring adjustments in view of the concepts of geriatrics, as well as the greater strengthening of actions in primary care, in order to reduce reflexes of limitation in the urgency and emergency service. It is expected, through the results found, to contribute to the development of a restructuring of this model, with a service that prioritizes the health needs of the elderly.
