Pharmacological effects of the association between antihypertensives and non-steroid anti-inflammatory agents: an updated review

Resumo: Systemic Arterial Hypertension (SAH) is considered a chronic multifactorial degenerative disease and a polygenic syndrome. Antihypertensive pharmacotherapy usually starts with first-line antihypertensive medications as monotherapy or in combination, which may be preferable in elderly patients with higher blood pressure (BP) levels. Aging is also accompanied by a considerable increase in the use of other medications, highlighting the use of analgesic and anti-inflammatory drugs, such as non-steroidal anti-inflammatory drugs (NSAIDs). Therefore, the aim of this review is to demonstrate the possible interactions between antihypertensives and NSAIDs, as well as highlight the main adverse reactions caused by such interactions, according to data published in the last five years. A review of the scientific literature, national and international, which contained in their titles and/or abstracts the following descriptors was carried out: systemic arterial hypertension, antihypertensives, anti-inflammatory drugs, NSAIDs. The use of multidrug therapy brings constant concern about drug interactions and the adverse effects that result from it. The analyzed studies report the use of antihypertensives (mainly inhibitors of the renin-angiotensin system) associated with NSAIDs and their consequences, such as cardiovascular diseases, nephrotoxicity and hyperkalemia. It is believed that the rational and adequate prescription of these drugs can minimize the adverse effects resulting from drug interactions.

La farmacoterapia antihipertensiva generalmente comienza con medicamentos antihipertensivos de primera línea como monoterapia o en combinación, lo que puede ser preferible en pacientes de edad avanzada con niveles más altos de presión arterial (PA). El envejecimiento también se acompaña de un aumento considerable del uso de otros medicamentos, destacando el uso de analgésicos y antiinflamatorios, como los antiinflamatorios no esteroideos (AINE). Por tanto, el objetivo de esta revisión es demostrar las posibles interacciones entre antihipertensivos y AINE, así como destacar las principales reacciones adversas provocadas por dichas interacciones, según datos publicados en los últimos cinco años. Se realizó una revisión de la literatura científica, nacional e internacional, que contenía en sus títulos y / o resúmenes los siguientes descriptores: hipertensión arterial sistémica, antihipertensivos, antiinflamatorios, AINE. El uso de la terapia con múltiples fármacos genera una preocupación constante acerca de las interacciones farmacológicas y los efectos adversos que se derivan de ella. Los estudios analizados informan del uso de antihipertensivos (principalmente inhibidores del sistema renina-angiotensina) asociados a los AINE y sus consecuencias, como enfermedades cardiovasculares, nefrotoxicidad e hiperpotasemia. Se cree que la prescripción racional y adecuada de estos medicamentos puede minimizar los efectos adversos resultantes de las interacciones medicamentosas.A Hipertensão Arterial Sistêmica (HAS) é considerada uma doença degenerativa multifatorial crônica e uma síndrome poligênica. Normalmente, a farmacoterapia anti-hipertensiva começa com medicamentos anti-hipertensivos de primeira linha em monoterapia ou em combinação, que pode ser preferível em pacientes idosos com níveis mais elevados de pressão arterial (PA). O envelhecimento também é acompanhado de considerável aumento do uso de outros medicamentos, destacando-se o uso de drogas analgésicas e anti-inflamatórias, tais como anti-inflamatórios não-esteroides (AINEs). Portanto, o objetivo desta revisão é demonstrar as possíveis interações entre anti-hipertensivos e AINEs, bem como destacar as principais reações adversas ocasionadas por tais interações, de acordo com dados publicados nos últimos cinco anos. Foi realizada uma   revisão   da   literatura   científica, nacional   e internacional, que contivessem em seus títulos e/ou resumos os seguintes descritores:  hipertensão arterial sistêmica, anti-hipertensivos, anti-inflamatórios, AINEs. O uso de poliquimioterapia traz uma preocupação constante sobre a interação medicamentosa e os efeitos adversos que decorrem dela. Os estudos analisados reportam o uso de anti-hipertensivos (principalmente inibidores do sistema renina-angiotensina) associados a AINEs e suas consequências, como doenças cardiovasculares, nefrotoxicidade e hipercalemia.
