Pharmacological profile of antihypertensive prescriptions and adequacy to the Brazilian hypertension guideline

Resumo: A frequent monitoring of the blood arterial pressure, associated with an adequate drug prescription for patients with systemic arterial hypertension, proves to be a challenge when it comes to standardized therapeutic recommendations for the control of hypertension. The present study aimed to show the pharmacological profile of the antihypertensive drugs use by patients in treatment at health units and the adequacy of the prescriptions to the Brazilian hypertension guideline. This is a cross-sectional study, from secondary data present in patient medical records. The data were compared with recommendations for antihypertensive prescriptions present in the Brazilian hypertension guidelines, and analyzes of absolute and relative frequencies were described. It was observed that diuretics and ACE inhibitors were the most prescribed drug classes. Prescription of only one antihypertensive was more prevalent among those patients with up to 5 years of hypertension diagnosis. Divergences with the guideline recommendations were observed, mainly, in prescriptions involving two or more medications, representing a percentage of 16% of them.

El presente estudio tuvo como objetivo mostrar el perfil farmacológico del uso de antihipertensivos por pacientes en los establecimientos de salud y la adecuación de las prescripciones a la directriz brasileña de hipertensión. Se trata de un estudio transversal, basado en datos secundarios de registros médicos de pacientes seguidos por el programa HIPERDIA. Se realizó una comparación entre los datos obtenidos y las recomendaciones de prescripción de antihipertensivos contenidas en la directriz brasileña de hipertensión arterial, y se describieron análisis de frecuencias absolutas y relativas. Los diuréticos y los inhibidores de la ECA representaban las clases de fármacos más recetados. La prescripción de un solo antihipertensivo fue más prevalente entre aquellos pacientes con hasta 5 años de diagnóstico de hipertensión. Se observaron divergencias con las recomendaciones de la directriz, principalmente, en prescripciones de dos o más medicamentos, lo que representó un porcentaje del 16% de estas recetas.O monitoramento contínuo da pressão arterial, associado a uma adequada prescrição medicamentosa a pacientes com hipertensão arterial sistêmica, mostram ser um desafio quando se trata de recomendações terapêuticas padronizadas no controle da hipertensão. O presente estudo teve como objetivo descrever o perfil farmacológico da utilização de anti-hipertensivos por pacientes de unidades de saúde e a adequabilidade das prescrições à diretriz brasileira de hipertensão. Trata-se de um estudo transversal, realizado a partir de dados secundários de prontuários de pacientes acompanhados pelo HIPERDIA. Os dados foram comparados com as recomendações de prescrição de anti-hipertensivos contidas na Diretriz Brasileira de Hipertensão Arterial, sendo descritas análises de frequências absoluta e relativa. Foi observado que diuréticos e inibidores da ECA representaram as classes medicamentosas mais prescritas. A prescrição de apenas um anti-hipertensivo foi mais prevalente entre aqueles pacientes com até 5 anos de diagnóstico de hipertensão.
