Surgical treatment of submandibular salivary gland sialolith: case report

Resumo: Sialolithiasis is one of the most common diseases of the salivary glands. Sialoliths usually occur in the submandibular ducts or gland and are classified as being of unusual size or “giant” sialoliths if any dimension exceeds 15 mm. The aim of this work is to report a clinical case of sialolith located in the submandibular gland, as well as to discuss the pertinent literature on the subject. A 68-year-old female patient attended the CTBMF service at the Trauma Hospital of Campina Grande, complaining of discomfort and episodes of pain, redness in the left submandibular region and dry mouth. A calcified mass measuring 16 x 10 mm was evidenced by computed tomography, in the path of the duct of the left submandibular gland. Salivary calculus excision was performed under general anesthesia. After accessing and removing it, a jelco nº 16 was installed to help re-epilate the site in order to maintain the patency of the involved duct. Therefore, the correct evaluation, considering the location and size of the sialolith, is indispensable for the treatment plan and the good execution, in this case, the surgical excision of the sialolite was the technique considered and positive results were obtained.

Los sialolitos suelen aparecer en los conductos o glándulas submandibulares y se clasifican como de tamaño inusual o sialolitos "gigantes" si alguna dimensión supera los 15 mm. El objetivo de este trabajo es reportar un caso clínico de sialolito localizado en la glándula submandibular, así como discutir la literatura pertinente sobre el tema. Paciente de sexo femenino de 68 años que acude al servicio de CTBMF del Hospital de Trauma de Campina Grande, que consulta por molestias y episodios de dolor, enrojecimiento en la región submandibular izquierda y sequedad de boca. Por tomografía computarizada se evidenció una masa calcificada de 16 x 10 mm, en el trayecto del conducto de la glándula submandibular izquierda. La escisión del cálculo salival se realizó bajo anestesia general. Tras acceder y retirarlo, se instaló un jelco nº 16 para ayudar a re-depilar el sitio con el fin de mantener la permeabilidad del conducto afectado. Por tanto, la correcta evaluación, considerando la ubicación y tamaño del sialolito, es indispensable para el plan de tratamiento y la buena ejecución, en este caso, la escisión quirúrgica de la sialolita fue la técnica considerada y se obtuvieron resultados positivos.A sialolitíase é uma das doenças mais comuns das glândulas salivares. Os sialólitos geralmente ocorrem nos ductos submandibulares ou na glândula e são classificados como de tamanho incomum ou sialólitos “gigantes” caso qualquer dimensão exceda 15 mm. Esse trabalho tem por objetivo relatar um caso clínico de sialólito localizado em glândula submandibular, bem como discutir a literatura pertinente sobre o tema. Paciente do sexo feminino, 68 anos, compareceu serviço de CTBMF do Hospital de Trauma de Campina Grande, queixando-se de incômodo e episódios dor, vermelhidão em região submandibular esquerda e boca seca. Uma massa calcificada com medidas 16 x 10 mm foi evidenciado através de uma tomografia computadorizada,  no trajeto do ducto da glândula submandibular esquerda. Foi realizado a exérese do cálculo salivar, sob anestesia geral. Após acesso e remoção do mesmo, foi feita a instalação de um jelco nº 16 para auxiliar a reepetilização do local a fim de manter a patência do ducto envolvido.
