Efficacy of medical grade honey in the treatment of diabetic foot ulcers: a systematic review

Resumo: Objective: To analyze the effectiveness of medical grade honey in the treatment of diabetic foot ulcers. Methodology: This is a systematic review carried out using the PRISMA method, in the electronic databases PubMed, Scopus and Web of Science, using the following descriptors: “honey”, “wound”, “wound healing”, “honey dressing” and “diabetic foot”. Full articles, available in Portuguese, English and Spanish, published between January 2010 and December 2020, research conducted in humans and studies focusing on honey in the treatment of diabetic foot ulcers were included. Results: Seven articles made up the final sample of this study, with the highest production in China (n: 03). All studies used the Randomized Clinical Trial methodology, characterizing the studies as presenting a strong level of evidence (II). The treatment of diabetic foot ulcers with honey was found to have good overall efficacy, such that four studies had higher healing rates compared to their respective control groups and shorter healing times. It was found that the results presented suggested that the honey dressing was associated with a higher rate and earlier bacterial clearance time compared to other dressings. Conclusion: It was found that medical grade honey was effective in the treatment of diabetic foot ulcers, promoting high healing rates in a shorter period and high bacterial load reduction. However, some coverages used as controls showed greater efficacy, prompting more robust studies for a greater generalization of the results.

Metodología: Se trata de una revisión sistemática realizada mediante el método PRISMA, en las bases de datos electrónicas PubMed, Scopus y Web of Science, utilizando los siguientes descriptores: “honey”, “wound”, “wound healing”, “honey dressing” y “diabetic foot”. Se incluyeron artículos completos, disponibles en portugués, inglés y español, publicados entre enero de 2010 y diciembre de 2020, investigaciones realizadas en humanos y estudios centrados en la miel en el tratamiento de las úlceras del pie diabético. Resultados: Siete artículos conformaron la muestra final de este estudio, con la mayor producción en China (n: 03). Todos los estudios utilizaron la metodología de Ensayo Clínico Aleatorizado, caracterizándose los estudios como de fuerte nivel de evidencia (II). Se encontró que el tratamiento de las úlceras del pie diabético con miel tiene una buena eficacia general, de modo que cuatro estudios tuvieron tasas de curación más altas en comparación con sus respectivos grupos de control y tiempos de curación más cortos. Se encontró que los resultados presentados sugirieron que el apósito de miel se asoció con una tasa más alta y un tiempo de eliminación de bacterias más temprano en comparación con otros apósitos. Conclusión: Se encontró que la miel de grado médico fue efectiva en el tratamiento de las úlceras del pie diabético, promoviendo altas tasas de curación en un período más corto y una alta reducción de la carga bacteriana. Sin embargo, algunas coberturas utilizadas como controles mostraron una mayor eficacia, lo que motivó estudios más robustos para una mayor generalización de los resultados.Objetivo: Analisar a eficácia do mel de grau médico no tratamento de úlceras de pé diabético. Foram incluídos artigos completos, disponíveis em português, inglês e espanhol, publicados entre janeiro de 2010 a dezembro de 2021, pesquisas conduzidas em humanos e estudos que concentrassem o mel no tratamento de úlceras de pé diabético. Resultados: Sete artigos compuseram a amostra final deste estudo, com maior produção da China (n: 03). Todas as pesquisas utilizaram a metodologia do Ensaio Clínico Randomizado, caracterizando as pesquisas como apresentando nível de evidência forte (II). Verificou-se que o tratamento de úlceras de pé diabético com mel obteve uma boa eficácia geral, de tal modo que quatro estudos apresentaram taxas de cicatrização maiores em relação aos seus respectivos grupos controles e tempo de cicatrização menores. Constatou-se que os resultados apresentados sugeriram que o curativo de mel estava associado a uma taxa maior e um tempo de eliminação bacteriana mais precoce em comparação com outras coberturas. Conclusão: Verificou-se que o mel de grau médico apresentou eficácia no tratamento de úlceras de pé diabético, promovendo taxas elevadas de cicatrização em um menor período e elevada redução de carga bacteriana.
