Patient medical relationship in the context of HIV/AIDS within the Brazilian Prison System: a brief bibliographic review

Resumo: | Currently, Brazil has more than 700 thousand people in prisons. In this regard, it is clear that the AIDS epidemic in Brazilian prisons is a major public health problem. In this perspective, among the aggravating and proliferating factors of the disease, the overcrowding of prisons, unprotected sexual practices and the unhealthiness of cells stand out. In this study, the Bireme, Scielo, Pubmed and Google Scholar research bases were used, and the descriptors used were: “health care”, “HIV (acquired immunodeficiency syndrome)” and “prisoners and prisons”. The objective is to answer the question: "How does the doctor-patient relationship take place in the care provided to people with HIV in the prison system?". The implementation of the National Policy for Comprehensive Health Care for Persons Deprived of Liberty in the Prison System (PNAISP) was also analyzed, which presumes comprehensive, continuous and resolutive assistance with control and reduction of injuries to prison health demands. As for the doctor-patient relationship, there are difficulties in care, due to the permanence of the stigma related to HIV / AIDS and the retrograde prevalence of the use of the biomedical model, known for focusing only on the disease, by health professionals, such adversities decrease both adherence and treatment effectiveness. Therefore, a more humanistic and welcoming doctor-patient relationship on the part of the health professional is of utmost importance, thus having greater chances of success in therapy and in the interpersonal relationship with the sick. |

En este contexto, es notorio que la epidemia de SIDA en las cárceles brasileñas es un problema importante de salud pública. Bajo este escenario, se destacan entre los agravantes y proliferantes de enfermedad, el hacinamiento de las prisiones, las prácticas sexuales desprotegidas y la insalubridad de las celdas. En este estudio, se utilizaron las bases de investigación Bireme, Scielo, Pubmed y Google Scholar, y los descriptores fueron: "cuidado de la salud", "VIH- (síndrome de inmunodeficiencia adquirida)" y "prisioneros y prisiones". El objetivo es responder a la pregunta: "¿Cómo se produce la relación médico-paciente en la atención prestada a las personas con VIH en el sistema penitenciario?". También analizamos la implementación de Política Nacional de Atención Integral de la Salud de las Personas Privadas de Libertad en el Sistema Penitenciario (PNAISP), que presume de atención integral, continua y resolutiva con control y reducción de lesiones a las demandas de salud penitenciaria. Delante a la relación médico-paciente, existen dificultades en la atención, debido a la permanencia del estigma relacionado con VIH/SIDA y la prevalencia retrógrada del uso de modelo biomédico, conocido por centrarse únicamente en la enfermedad, por parte de los profesionales de salud, tales adversidades disminuyen tanto la adversidad como la eficacia del tratamiento. Por lo tanto, una relación médico-paciente más humanista y acogedora por parte del profesional de la salud es de suma importancia, para tener una mayor probabilidad de éxito en la terapia y las relaciones interpersonales con los enfermos.Atualmente o Brasil possui mais de 700 mil pessoas em cárcere privado. Nesse viés, é notório que a epidemia da AIDS, nas prisões brasileiras, é um grande problema de saúde pública. Nessa perspectiva, destacam-se, entre os agravantes e proliferadores da doença, a superlotação das prisões, as práticas sexuais desprotegidas e a insalubridade das celas. Nesse estudo foram utilizadas as bases de pesquisas Bireme, Scielo, Pubmed e Google Acadêmico, e os descritores usados foram: “assistência à saúde”, “HIV (Síndrome da Imunodeficiência Adquirida)” e “prisioneiros e prisões”. O objetivo é responder à pergunta: “Como acontece à relação médico-paciente na assistência prestada às pessoas portadoras de HIV no sistema prisional?”. Analisou-se, também, a implementação na Política Nacional de Atenção Integral à Saúde das Pessoas Privadas de Liberdade no Sistema Prisional (PNAISP), a qual presume uma assistência integral, contínua e resolutiva com controle e redução dos agravos as demandas de saúde no sistema prisional. Quanto à relação médico-paciente, constatam-se dificuldades no atendimento devido à permanência do estigma referente à HIV/AIDS e a prevalência retrógrada da utilização do modelo biomedicinal, conhecido por focar apenas na doença, por parte dos profissionais da saúde, tais adversidades diminuem tanto a adesão quanto à efetividade do tratamento.
