Nursing workload of adult patients hospitalized in an emergency room of a high complexity teaching hospital

Resumo: Introduction: Recognizing the types of care and workload and their oscillation in time in high complexity emergency rooms and their determinants are essential in care management. Despite this there is a scarcity of these data in the literature. Objective: Evaluating the nursing workload of adult patients hospitalized in 2013 and 2019 in different sectors of an emergency room of a teaching hospital of high complexity and open door. Methodology: The types of care, the degree of complexity and the scores of the instrument of the Santos Patient Classification System for adult patients admitted to the emergency room as a function of the subunit were evaluated. Results: The type of care was dependent on the year of sampling for the General Emergency Room, with the high dependence predominated (X2 = 11.43; d.f. = 4; p = 0.022) and for the Clinical Emergency Room with a predominance of intensive care (G = 15.45; d.f. = 4; p = 0.004); while it was not dependent on Clinical-Surgical hospitalization, with the intermediate predominated (G = 1.11; d.f. = 4; p = 0.892). The type of care oscillated between the subunits and reflected the care provided in these units, evidenced when the degree of complexity of care in each subunit was evaluated. Discussion and Conclusion: The results demonstrated the need to evaluate each subunit of the emergency room independently and in time, since they presented different workload profiles. This reflects the specialization, organizational and administrative structure of the emergency room in high complexity units; with a strong impact on patient safety.

A pesar de esto, hay una escasez de estos datos en la literatura. Objetivo: Evaluar la carga de trabajo de enfermería de pacientes adultos hospitalizados en 2013 y 2019 en diferentes sectores de una sala de emergencias de un hospital universitario de alta complejidad y puertas abiertas. Metodología. Se evaluaron los tipos de atención, el grado de complejidad y las puntuaciones del instrumento del Santos Sistema de Clasificación de Pacientes para pacientes adultos ingresados en urgencias en función de la subunidad. Resultados: El tipo de atención dependió del año de muestreo para la sala general de emergencias, predominantemente alta dependencia (X2 = 11,43; g.l. = 4; p = 0,022) y para la Sala de Emergencias Clínicas con predominio de cuidados intensivos (G = 15,45; g.l. = 4; p = 0,004); mientras que no fue dependiente de la hospitalización Clínico-Quirúrgica, predominando el intermedio (G = 1,11; g.l. = 4; p = 0,892). El tipo de atención osciló entre las subunidades y reflejó la atención prestada en estas unidades, evidenciada cuando se evaluó el grado de complejidad de la atención en cada subunidad. Discusión y Conclusión: Los resultados demostraron la necesidad de evaluar cada subunidad de la sala de emergencias de manera independiente y en el tiempo, ya que presentaron diferentes perfiles de carga de trabajo. Esto es un reflejo de la especialización, estructura organizativa y administrativa de la sala de emergencias en unidades de alta complejidad; con un fuerte impacto en la seguridad del paciente.Introdução: Conhecer os tipos de cuidado e a carga de trabalho e sua oscilação no tempo em prontos-socorros de alta complexidade e seus determinantes são essenciais na gestão do cuidado. Apesar disto há uma escassez desses dados na literatura. Objetivo: Avaliar a carga de trabalho em Enfermagem de pacientes adultos internados em 2013 e 2019 em diferentes setores de um pronto-socorro de um hospital de ensino de alta complexidade e porta aberta. Metodologia: Foram avaliados os tipos de cuidados, o grau de complexidade e os escores do instrumento do Sistema de Classificação de Pacientes de Santos para os pacientes adultos internados no pronto-socorro em função da subunidade. Resultados: O tipo de cuidado foi dependente do ano de amostragem para o Pronto-socorro Geral predominando alta-dependência (X2 = 11,43; g.l. = 4; p = 0,022) e para a Sala de Emergência Clínica predominando o intensivo (G = 15,45; g.l. = 4; p = 0,004); enquanto não foi dependente para o a internação Clínico-Cirúrgica predominando o intermediário (G = 1,11; g.l. = 4; p = 0,892). O tipo de cuidado oscilou entre as subunidades e refletiu a assistência prestada nestas, evidenciado quando avaliados o grau de complexidade do cuidado em cada subunidade. Discussão e Conclusão: Os resultados demonstraram a necessidade de avaliação de cada subunidade do pronto-socorro independentemente e no tempo, já que apresentaram diferentes perfis de carga de trabalho.
