Onychomycoses due to fungi of the genus Candida: a literature review

Resumo: Onychomycosis are superficial infections that are characterized by the destruction of the nail bed and adjacent tissues with yeast fungi as important etiological agents of these diseases. In this context, the present study aimed to conduct a literature review on the genus Candida, emphasizing the main species responsible for onychomycosis, according to their epidemiological, pathogenic, immunological, diagnostic and pharmacological treatment. The research was developed in an exploratory and qualitative way, with a database approach of scientific publications, repositories and indexers. In view of the articles, dissertations, books and theses, C. albicas, C. tropicalis and C. parapsilosis has been presented as the main species of the genus Candida from a clinical and epidemiological, for nail infections with diagnosis centered on the clinical history of the laboratory tests, and more recently, by the use of image recording. The infection and dissemination process in nails occurs when virulence factors (adhesion, biofilms, pleomorphism, toxins, hydrolytic enzymes and phenotypic variability), overlap the defensive factors (microbiota, mechanical barriers and immune system). The treatment is based on the eradication of the infecting microorganism and regression of signs and symptoms, with three main pharmacological strategies, topical treatment (amorolfilina, ciclopirox), oral treatment (itraconazole and fluconazole) and combination therapy, however resistance mechanisms have been associated with these drugs. Given the above, the information present in this research serves as an aid for the consolidation of clinical - epidemiological knowledge that focuses on onychomycosis, caused by fungi of the genus Candida, including students, health professionals, academics and all society.

En este contexto, el presente estudio tuvo como objetivo realizar una revisión de la literatura sobre el género Candida, enfatizando las principales especies responsables de la onicomicosis, de acuerdo con su tratamiento epidemiológico, patógeno, inmunológico, diagnóstico y farmacológico. La investigación se desarrolló de forma exploratoria y cualitativa, con un enfoque de base de datos de publicaciones científicas, repositorios e indexadores. En vista de los artículos, disertaciones, libros y tesis, C. albicas, C. tropicalis y C. parapsilosis se ha presentado como la principal especie del género Candida desde un punto de vista clínico y epidemiológico, para infecciones ungueales con diagnóstico centrado en la historia clínica de las pruebas de laboratorio, y más recientemente, mediante el uso de grabación de imágenes. El proceso de infección y diseminación en las uñas ocurre cuando los factores de virulencia (adhesión, biopelículas, pleomorfismo, toxinas, enzimas hidrolíticas y variabilidad fenotípica) se superponen a los factores defensivos (microbiota, barreras mecánicas y sistema inmunitario). El tratamiento se basa en la erradicación del microorganismo infeccioso y la regresión de los signos y síntomas, con tres estrategias farmacológicas principales, tratamiento tópico (amorolfilina, ciclopirox), tratamiento oral (itraconazol y fluconazol) y terapia combinada, sin embargo, los mecanismos de resistencia se han asociado con estas drogas. Dado lo anterior, la información presente en esta investigación sirve como una ayuda para la consolidación del conocimiento clínico - epidemiológico que se centra en la onicomicosis, causada por hongos del género Candida, incluidos estudiantes, profesionales de la salud, académicos y toda la sociedad.As onicomicoses são infecções superficiais que se caracterizam pela destruição de leito ungueal e tecidos adjacentes, apresentando fungos leveduriformes como importantes agentes etiológicos dessas afecções. Nesse contexto, o presente estudo teve como objetivo realizar uma revisão de literatura sobre o gênero Candida, enfatizando as principais espécies responsáveis pelas onicomicoses, conforme com suas características epidemiológicas, patogênicas, imunológicas, diagnóstico e o tratamento farmacológico. A pesquisa foi desenvolvida de forma exploratória de caráter qualitativo, com abordagem em base de dados de publicações científicas, repositórios e indexadores. Diante dos artigos, dissertações, livros e teses, C. albicas, C. tropicalis e C. parapsilosis tem se apresentado como as principais espécies do gênero Candida do ponto de vista clínico e epidemiológico, para infecções em unhas com diagnóstico centrado no histórico clinico do paciente, exames laboratoriais, e mais recentemente, pela utilização de registro de imagens. O processo de infecção e disseminação em unhas ocorre quando os fatores de virulência (aderência, biofilmes, pleomorfismo, toxinas, enzimas hidrolíticas e variabilidade fenotípica), se sobrepõem aos fatores defensivos (microbiota, barreiras mecânicas e sistema imunológico). O tratamento está baseado na erradicação do microrganismo infectante e na regressão de sinais e sintomas, com três estratégias farmacológicas principais, o tratamento tópico (amorolfilina, ciclopirox), tratamento oral (itraconazol e fluconazol) e terapia combinada, todavia fenômenos de resistência já têm sido associados a esses fármacos.
